Goals for the first session

1. Put them at ease

2. Establish rapport

3. Let them know what to expect

4. Emphasize the importance of everybody there

5. Get there commitment to return before they leave

Stages of the initial interview

1. Preinterview: (usually over the telephone)

*set expectations for attendance and gathering information

2. Social: (centered around chitchat....usually on the way to the office)

*address this phase by following the hierarchy

3. Problem Definition: (understanding of the problem from everyone)

*don't try to change thoughts/opinions...just listen

* often a hitch: (avoidance of mentioning usage) occurs here

4. Interaction: (having family interact with each other)

* focus on strengths and stay on topic

5. Closing: (summarize/motivation for return/ continue to point out strengths/ think about referrals, ask for 3 session commitment/ set preliminary goals)

First session

*easier emotionally for a therapist but session requires structure (keep everyone in play)

* therapist needs to keep everyone on focus

* do not address enabling/ denial at this time

* do not try to play expert but listen to the family

Reasons why people might drop out of treatment

"Therapist needs to listen to the family before the family listens to the therapist"-John T. Edwards

1) The crisis subsides

2) the family doesn't think they need additional treatment

3) want to protect themselves from having to go deeper

4) they are confused about how therapy works
