What is Tian Gui? Where does it originate from?
Tian Gui is menstrual blood and ova from ovaries. In men it is sperm. It originates from the Kidneys
What other organs influence blood in women?
Liver & uterus = store bloodST & Sp = post-natal Qi and blood --> menstrual blood
What type of organ is the uterus (bao)?
It is one of the six extra Yang organs. Its yang function is to discharge menstrual blood and the act of labour. Its yin aspect stores blood and nourishes the fetus.
What is the channel (Bao luo) and vessel (Bao luo) of the uterus?
The uterus channel connect the uterus to the kidneys. The uterus vessel pertains to the heart and extends to the uterus.
At age 14 what is the physiology of women?
Tian Gui arrives, the Ren Mai (provides essence and yin) is open, the Chong Mai (provides and moves blood) is flourishing, the period arrives and conception is possible.
What is the status of Yin and Yang during the menstrual cycle? (esp Kidney Yin and Yang)
Yang decreases when menses starts and reaches the minimum at ovulation. Maximum of yang occurs shortly before menses.Yin is at its minimum level before menses and maximizes at ovulation and decreases until beginning of menses.
The times of transition from Yin to Yang or Yang to Yin is marked by what?
There is always discharge and downward movement.Yang --> Yin = mensesYin --> Yang = ovulation
What controls the yin-yang, yang-yin transformation?
Both transformations depend on the descent of Ht-Qi and Ht-Blood
What occurs during the menstrual phase?
Heart-Qi descends --> Qi & Blood move downward, level of Yang drops sharply while Yin grows.Liver-Blood and Chong Mai influences this phase.
What occurs during the follicular phase?
Blood and Yin are empty from menstrual phaseYin increases --> growth of fluids and cervical secretionsEssence matures --> nourishing uterus & follicles, growth of endometrium
What occurs during ovulation?
Yin reaches its maximum --> fertile cervical secretion (manifestation of Kd-Yin & Essence)The Du, Ren & Chong Mai must be flourishing for ovulation to occur.Heart Qi descends --> Yin transfroms to Yang --> discharge of ovum
What occurs during the luteal phase?
Liver-Qi & Liver-Blood --> Qi & Blood movesYang increases --> expels any potential yin pathogens in uterus (D-P, cold, Blood stasis)Chong Mai has strongest influence.
What are the treatment principles during the menstrual phase?
Influenced by Chong MaiInvigorate blood, stop bleeding (only if heavy menses)
What are the treatment principles during the follicular phase?
Influenced by Ren Mai & Kidney YinNourish Blood and tonify Kidneys
What are the treatment principles during ovulation?
Influenced by Ren & Du Mai, Kidney Yin & YangTonify the Kidneys, Stregthen Du, Ren, & Chong Mai, (if needed resolve dampness & tonify Spleen)
What are the treatment principles during luteal phase?
Influenced by Chong MaiMove Qi, invigorate blood, warm the uterus (if needed expel Cold, resolve Damp-phlegm)
What role does the Ming Men fire play?
The fire of Ming Men warms the uterus and balances Yin.Promotes maturation of follicles and the production of ova = makes conception possible
What role does the Minister fire play (Kidney fire)?
When Minister fire is xu = uterus is not warm --> infertility, dysmenorrhoea, low libidoWhen Minister fire is shi = heats the blood --> excessive menstrual bleeding, infertility, miscarriageMinister Fire + Kidney-water = Tian Gui
What role does the Liver play?
Blood stored in Liver = Tian Gui + Post-Natal Blood & Qi (Sp&ST)Health of Liver Qi is vital to movement of bloodWhen treating blood for women's physiology always treat Liver as wellIf Liver blood xu = secondary Liver Qi stagnation
What role does the Spleen play?
Spleen makes post-natal blood --> stored in Liver with Tian GuiLiver-Blood Xu = Tian Gui xu and/or Post-Natal blood xuSpleen-Qi keeps the uterus in placeSinking of Sp-Qi -->prolapse of uterus or bladder, menorrhagiaSpleen-Qi gathers Blood.Spleen Qi xu --> blood leaks = menorrhagia
What role does the heart play?
Heart governs bloodHeart is connected to uterus via the Bao maiHeart-Qi descent controls the yin->yang (ovulation) and yang->yin (menses) transformation during menstrual cycleIf Heart-Qi does not descend --> no ovulation, no mensesHeart-blood xu --> scanty periods or amenorrhoeaHeart-blood stasis --> painful periodsHeart-blood heat --> menorrhagia
What role does the lung play?
Minor influence, connected to Spleen Qi xuSaddness or grief --> depletion of Qi = no menses
What role does Stomach play?
Chong Mai arises from the uterus, passes through ST 30 and the StomachChong Mai <-> ST --> morning sicknessST flows through the breast & supplies Post-natal Qi --> breast milkST and Sp --> source of post-natal qi & blood
Role of Chong Mai
Chong Mai strongly influences menstruation = Sea of Blood--> influences the supply and proper movement of Blood in Uterus--> works with Ren Mai to regulate Uterus and menstruation--> nourishes and moves Blood
Role of Ren Mai
Ren Mai is closely related to Uterus, internal & external genitaliaDysfunction of Ren Mai --> problems with cervix, vagina & vulvaRen Mai --> provides Yin substances (Essence, Blood & fluids) for all women's physiological processes and hormonal gateways
Role of Du Mai
The Du Mai and Ren Mai are almost one vessel.Du mai --> tide of Yang during menstrual cycleRelated to the Ming Men and Minister Fire
Role of Dai Mai
Encircles leg channels --> restrains Liver's flow of Qi, harmonizes vertical movement of Spleen & KidneysThe functions & structures of the Kidney, Spleen & Liver rely on Dai Mai being relaxed and stretchedIf Dai Mai not restricted enough --> Damp-Heat --> leucorrhoeaIf Dai Mai xu --> Ren MaiDai Mai xu -->Kd & Liv xu -->Essence rebels, Sp Qi sinks, unsupported Post-Natal Qi --> Du, Ren, Chong Mai XuIf Dai Mai shi --> Chong Mai
What are the requirements for conception?
The ingredientsabundant supply of Blood & Essenceflourishing of Kidneys & Liverhealthy Chong & Ren MaiThe 'spark'Ming MenAdd in the personalFather's essenceMother's essence + Corporeal Soul (Po)
What happens to Yin & Yang during pregnancy?
Yin increases --> no longer monthly loss of blood + build up of amniotic fluidYang increases --> no more menses = build up of Minister Fire
What happens to blood in the first trimester of pregnancy?
Blood --> moves to upper burner --> milk & breast distensionAs a result --> Qi rebels in the Chong Mai --> morning sickness
What effect does pregnancy have on the Kidneys?
Kidneys strained --> Qi & Essence feeds fetusMenstrual blood (Tian Gu) --> Essence that nourishes body & fetus
What causes the increase in heat during pregnancy?
Emperior & Minister Fire --> nourishes fetus == feeling of heat and restlessness in mother.
What occurs during Menopause?
Decline of Kidney energy, the Ren & Chong MaiDry of the fluids & Blood
What is the luteal phase defect?
It is defined as a Luteal phase that is shorter then 10 days.It occurs because of poor follicle development ORpremature failure of the corpus luteum ORfailure of uterine lining to respond
How does Chinese Medicine view the Luteal phase defect?
Yang energy increases during the Luteal phase1. too little yin in follicular phase --> too little yang in luteal phase2. Stagnation at ovulation --> prevents transition from Yin to Yang3. Too little Yang --> prevents ovulation and/or shorten Luteal phase
How do you define late periods?
Late up to one week (40 to 50 day cycle) for two consecutive cycles
What are the two different types of painful period?
Primary = common in young womenSecondary = ex. endometriosisThe main pathology is Qi and blood stagnation
What should you rule out in painful periods?
It could be an ectopic pregnancy. This is an emergent condition
How should herbs be modified for painful periods?
According to Cold, Qi stagnation, Blood stasis and heat
When should you treat for painful periods?
Several days before and during menses
What is endometriosis?
The presence of endometrium tissue outside the uterus. Painful periods, infertility and pelvic pain are the most common outcomes.
What are the three steps to treating flooding and trickling?
1. Shore up the flow2. Settle the source3. Restore the old
What are the four treatment strategies for stopping bleeding?
1. Harmonize the blood2. Treat the root3. Astringe4. Treat the Qi
What is Zang organ disorder?
visceral restlessness"saddness and tends to weep constantly as if haunted. Stretching frequently and yawns repeatedly"
What is Lily bulb disease?
as if in a trance" or absent-mindedPatient wants to eat but is relectant to swallowunwilling to speakhe or she wants to lie in but cannot lie quietlycannot tolerate the smell of foodwants to walk but soon tired"
What are the seven emotions of Chinese medicine?
1. Joy --> Heart2. Anger --> Liver --> ST&Sp3. Anxiety --> Lungs & LI4. Pensiveness --> Spleen5. Grief --> Lungs --> Qi6. Fear --> Kidney7. Fright --> Heart --> Kd
What is the role of Kidneys in menopausal syndrome?
Menopausal symptoms are due to a decline of Kidney-Essence in its Yin or Yang aspect.Often these deficiencies are combined with excess of Dampness, Qi stagnation, Blood stasis, Empty-Heat or Liver-Yang rising
How to teach menopausal women about osteoporosis?
Osteoporosis = low estrogen>weight-bearing exercise>calcium supplement with vit D>Tonify Kidney essence and yang with strengthen bone herbs
Teach menopausal women about coronary heart disease.
#NAME?
How does infertility happen from the Chinese medicine view?
1. constitutional weakness of Kd essence2. overwork --> Kd-Yin xu3. Excessive physical work --> Sp or Kd Yang xu4. Sexual activity too early --> weakens Kd and damage Ren and Chong Mai5. Invasion of Cold --> Obstructs uterus, Ren and Chong Mai6. Irregular diet --> Cold = as above, Phlegm = blockages (uterus, fallopian tubes)
