if asked about LIVER problems and parasites, which should be considered and why?
entomeba histolotica - remember that invasion into the intestines is quickly followed by metastasis to the liver through the portal circulation.

leishmania - it's a macrophage-infecting disorder, and in the VL (visceral leishmania) form of the disease (as opposed to CL, DCL, MCL), it routinely causes hepato/splenomegaly.

splenomegaly/hepatomegaly can be malaria.

also, ascaris roundworms can migrate into the liver and cause liver abscesses when scared - though you more often think about the ampule of vader/cholycistitis/pancreas

visceral larva migrans has HEPATO/SPLENOmegaly as bolded concerns - the worms migrate quickly and form eosinophelic granulomas all over.

echinococcus granulosis (hydatid disease) tends to make big cysts in the liver and lungs
what parasites might be involved with CNS problems?
malaria - specifically P. falciparum.

toxoplasma

either of the trypanosomes (the kinetoplastids, which include trypanosomas rhodesie/gambiese, which cause sleeping sickness, or chagas disease (cruzi).

trichonella, after awhile, can make it up there and cause encephalitis.
conjunctivitis and periorbital edema is likely...?
chagas disease! note that trichonella can also cause orbital edema.
if you're given a TZANCK smear that shows multinucleated giant cells, and also something about COWDRY inclusion bodies, what should you think of?
Cowdry bodies = Herpes.

Downey bodies = EBV.
what parasites invade macrophages specifically?
think of TOXO and think of LEISCHMANIA. remember that you get tachyzoites/bradyzoites based on cellular response, macrophage infection in tissue, etc.

with leischmania, macs collect in tissue to give you CL, or spread all over your body and give you VL.
what does orbital edema make you think of?
trichinella

also, CHAGAS disease! this is romana's sign in acute chagas infection.
