what tapeworm species should we be worried about and what are the animal hosts?

what are the parts of a tapeworm?
the scolex = head. Proglotid = segments. The worms are made of lots of proglotids: these each contain lots of babies that are male/female and can live independently (proglotids can come out your ass and crawl away). proglotids are hermarphroditic.
describe the lifecycle of the tapeworms, and the really weird thing about these.
really weird thing - cestodes use HUMANS AS THE DEFINITIVE HOST! sexual replication happens in people. Animal colonization = intermediate host.

People have the adult worm. It the proglotids are hermaphroditic, make lots of eggs, either the prlglotids full of eggs fall out or the eggs fall out the ass. Hence, PEOPLE ARE THE DEFINITIVE HOSTS OF TAPEWORMS.

live in the environment, eggs get eaten by pigs/cows, the "oncospheres" hatch, circulate around and end up making cysts in the muscles (feel in the tongue of animals).

Humans eat undercooked meat, get re-infected.


OR -
humans pass out the eggs, people eat people poop (take the animal's role) - become sorta intermediate hosts, the worm burrows out into our tissue and forms cysts, waiting to be eaten, never happens.
how is pathogenesis caused and not caused? symptoms?
having the adult stage worm from eating undercooked meat is generally not too dangerous. sure it can be up to 25 meters, usually only mild abdominal pain. maybe scary to see proglotids crawling out ass.

the LARVAL STAGE IS DANGEROUS - getting human-derived poop/eggs is dangerous, get NEURO-CYSTICERCOSIS. eggs = bad.

get cysticerci in BRAIN PARENCHYMA. while bug alive, no big deal. maybe block a ventricle, maybe have "mass effect'.

when it dies, stops hiding from the immune system and GET BIG IMMUNE RXN. SEIZURES common. headache. Maybe fatal.

left with calcified nodule that can be seizure focus.
risk factors?
geographic location - usually from immigrants in US. need to have exposure to human feces to have the neuro cysticocycosis. NIGHT SOIL. JEWS got it, not from eating pig, but from having food prepared by dirty immigrants. remember that eating undercooked animals will give you adults that aren't too dangerous.
diagnosis and treatment of tapeworms?
see eggs in poop. proglottids in poop good sign.

can see neurocycsticociss with serology.

treatments: for adult worms, can use praziquantel.

for neurocysts, use albendazole/praziquentel - but remember that the immune response is bad news, so you NEED TO GIVE CORTICOSTEROIDS to keep it from killing you.
