Systemic Mycoses

Primary Systemic Funguses
-endemic to specific locations (e.g., China, India, etc)--- some prefer low pH environments, others prefer high pH environments; some prefer humid areas, others prefer dry areas

-exogenous sources (not part of normal flora)

-can't be transmitted from person to person

-dimorphic--- exist as mycelia at room temp and as yeasts inside the host

-more prevalent in immunocompromised people (than in normal, healthy people)
Systemic Mycoses

Types of fungi that cause primary systemic mycoses (4)
1) Cocidioidomycosis

2) Histoplasmosis

3) Blastomycosis

4) Paracocidioidomycosis
Systemic Mycoses

Histoplasmosis
-fungi that cause primary systemic mycoses

-like all PRIMARY PATHOGENS, they enter the body via a primary complex and settle in the LUNGS, from where they can disseminate to the rest of the body

-enter the lungs/alveoli as conidia (rather than as mycelia), b/c mycelia are too large to enter the alveoli and are phagocytoses
Systemic Mycoses

________ and _______ are variants of Histoplasma Capsulatum.
-Var Capsulatum

-Var Debusie
Systemic Mycoses

Types of Histoplasmosis diseases (4)
1) Acute pulmonary histoplasmosis

2) Chronic pulmonary histoplasmosis

3) Disseminated histoplasmosis

4) African histoplasmosis
Systemic Mycoses

Acute Pulmonary Histoplasmosis
-occurs in immunocompromised patients (v. difficult to treat)
Systemic Mycoses

_______ and acute pulmonary plasmosis are indestinguishable when viewed under a microscopic.
-TB
Systemic Mycoses

Histoplasmin
-antibody secreted by histoplasmas during histoplasmosis (can be detected via serotological methods)

-similar appearance to TB antibodies (?)

-histoplasmins that are produced in pt's who had histoplasma for several years are indestinguishable from TB antibodies

-if pt is MALNOURISHED--> don't produce enough antibodies--> FALSE NEGATIVE results

-serotological tests that are used include: 1) Complement Fixation-immunological test with RBC, 2) Immuno-Deficient serotological test
