Tx: Empiric-vanco, ceftriaxone, ampicillin for listeria
Imaging after empiric: mri/ct
Ddx:
meningitis
encephalitis

Eti: GBS, E. coli, Listeria

What are the tests and Tx?
Sx: 37 yo m. sev headache 12 hr, px w/ head movement, nauseous, thinking "fuzzy"
Exam: T38.7 HR:104 RR20 BP 90/65
Nuchal rigidity, inability to communicate easily.

What are your Ddx and Eti?
Tests:
LP w/ opening press, gram stain, glucose, prot, culture
CBC w/diff
Metabolic panel
Blood gram stain and culture

Tx: Empiric Vanco and ceftriaxone
Imaging after, unless signs of intracranial pressure
Ddx: meningitis, encephalitis

Eti: S. pneumo, N. meningitis

What tests and Tx?
Sx: Inf <3 mo. Is Febrile/hypothermic, inconsolable, low urine output, apnea, decrease feeding, jaundice, poor color, lethargic
Exam: Temp >=38C (100.4),
Dehydrated, Ant font swollen, nasal flaring, hepatosplenomegaly, cap refill, petechiae, rashes, reflexes
Tests:
CBC w/diff
CMP
Blood/urine stain + culture
CRP
LP
Imaging: CXR, KUB, brain imaging
Ddx: UTI, bacteremia, meningitis, encephalitis, pneumonia, osteomyelitis, septic arthritis, skin and soft tissue inf, bacterial gastroenteritis,

Tx: Cefotoxime G+ and G-, if > 1mo. CSF penetration
Amp - G+ only
Consider: Vanc, acycl, naf
