Emergent surgical decompression, with excision of testicle if it infarcts

Unilateral testicular pain, dysuria, occasional urethral discharge, fever, leukocytosis in severe cases, painful & swollen epididymis

Epididymitis

Severe tenderness over superior pole of testicle, "blue dot" sign of ischemic appendage, normal position and lie, cremasteric reflex present, testicle & epididymis not tender

Appendix testis (torsion of testicular appendage)

U/S, perfusion confirmed with nuclear medicine scan
