Acute onset testicular pain & edema, n/v, tender, swollen testicle with transverse lie, absent cremasteric reflex on affected side

How is testicular torsion diagnosed?
Doppler U/S to assess testicular artery flow
How is testicular torsion treated?
Emergent surgical decompression, with excision of testicle if it infarcts
Unilateral testicular pain, dysuria, occasional urethral discharge, fever, leukocytosis in severe cases, painful & swollen epididymis
Epididymitis
Severe tenderness over superior pole of testicle, "blue dot" sign of ischemic appendage, normal position and lie, cremasteric reflex present, testicle & epididymis not tender
Appendix testis (torsion of testicular appendage)
How is appendix testis diagnosed?
U/S, perfusion confirmed with nuclear medicine scan
What is the tx for appendix testis?
Supportive, should resolve in 2 weeks
