DDx: viral myelitis

White matter - periphery of cord
Gray matter - central cord
Lupus-related myelitis
-High T1 and T2
-During flare-ups, cord enlargement in 75% (50% enhance)
-Resolution of cord swelling and enhancement during remission
-Eventual myomalacia
Neoplastic
-multiple lesions
-not much ligamentous enhancement
-posterior elements involved
Infection
-one vertebral body involved
-enhancement of ALL and PLL
Osteoporosis
-anterior wedging
