Open Neural Tube defects (NTDs)
MMC: Myelomeningocele (involves CNS tissues)

Donut shaped ring formed by stack verterbra. In OTC, have normal body, but no lamina or spinous processes. Normal spinal cord but flattened, so cord goes out back.

Neural tube (endoderm) doesn't close in itself. Non closed portion can be outside of body.

1/2000 or so births. Incidence reduced by 50% (folic acid supplements). Also early detection and possible termination.
Associated anomalies of NTDs.
Hydrocephalus
Type 2 Chiari malformation
Brainstem anomalies (medullary kink, downward displacement of medulla)
Urinary and bowel incontinence
Lower extremities paralysis/deformities (like club feet)

Buildup of spinal liquid in brain. Can use shunt to drain.
Type 2 Chiari
Herniation of brain (cerebellum)
Location of Neuro deficit versus need for shunt
Higher up deformity = less function = more morbidity/mortality/need for care.
Fixing NTDs
Neurosurgery/urology/orthopedics

Repair within 72 hours of birth. Prevents infection, one of main killers.

Can fix intrauterine!!
