Skill 1- pressure ulcer prevention

1- demonstrate two ways to prevent pressure ulcers


•proper use of bed cradle

Skill 2- Indwelling Foley Catheter Care

•don gloves


•when asked by examiner, verbalize need to: clean catheter tubing via cleansing wipe according to facility policy but NOT ALCOHOL) at least twice a day. Including peri care after each BM


•wash tubing 4 inches, beginning at urinary meatus and working downwards


•secure tubing to residents inner thigh or abdomen


•place tubing over leg


•position tubing to facilitate gravitational flow, meaning no kinks


•attach tubing to bed frame (not over side rail) and always below level of bladder


•when asked by examiner verbalize need to empty catheter bag frequently (according to facility policy or when bag is 1/2 full and over)


•keep bag from touching floor and provide privacy cover for bag


•remove gloves and wash hands


•document all catheter care

Skill 3 - Oxygen

•demonstrate correct placement of o2 nasal cannula


-place prongs following contour of nasal passage and tubing around ears and under chin not behind head


•when asked by examiner demonstrate how to check oxygen flow meter and verbalize actions needed if flow rate is not accurate


•never adjust flow of oxygen alert nurse immediately if incorrect


•verbalize 3 oxygen use guidelines


ex:


-avoid lighting matches or smoking around oxygen use


-ensure all electrical equipment is in good repair


-no kinks in tubing


-provide oral care frequently

Skill 4 - Occupied drawsheet change

•don gloves


•place clean draw sheet on clean surface within reach


•lower head of bed and place resident in supine position


•after raising side rail assist resident to their side moving them toward raised rail


•loosen drawsheet roll soiled draw sheet toward resident


•place and tuck clean draw sheet on working side (must be done before turning resident)


•raise side rail and assist resident to turn onto clean draw sheet


•remove soiled linens and draw sheet Avoid contact with your scrubs and place in appropriate location within room. Do not ever put on floor.


•pull and tuck in clean draw sheet finish with a sheet that is free of wrinkles


•Remove gloves and wash hands


*change gloves anytime they are soiled

Skill 5 - Applying cold compress

cover cold compress with towel or other protective cover (never place on bare skin without cover)


•properly place on correct site as directed by skills examiner


•when asked by examiner verbalize frequency of checks and how long you would leave compress on resident. Initially check after 5 mins. Do not leave on resident more than 20 mins


•asses the site for redness swelling irritation and pain if this occurs remove compress and report to nurse immediately

Skill 7 - Converting ounces to mLs

•convert oz to mLs


•Record amount accurately within + or - 25mLs of examiners reading


(30 mLs = 1 oz)


Skill 6 - Measure and Record Fluid Intake

•calculate intake in mL


•Measure on a flat, level surface


•Record intake accurately within + or - 25mLs of examiners reading

Skill 8 - Measure and Record Urine output

•don gloves


•measure the urinary output in a urinal/graduated container


•place on flat surface and measure accurately reading at eye level


•dispose of urine properly into toilet


•rinse container


•remove glovee wash hands


record output accurately within + or - 25 mLs of examiners reading

Skill 9 - Conscious Choking

•candidate is able to identify the symptoms of choking asks resident "are you choking"


•when asked by examiner verbalize need to call for help


•stands behind resident and wraps arms around residents waist


•places thumb side of the fist against residents abdomen


position fist slightly above navel


• grasp fist with other hand press fist and hand into residents abdomen with an inward upward thrust


•candidate should indicate that they would repeat this procedure until its successful or until victim loses consciousness

Skill 10 - Obtain and record Weight and Height

Weight-Standing Scale:


move weights to zero before assisting resident onto scale


•assist resident on scale


•ensure resident is balanced and centered on scale with their arms at their side


accurately record weight within + or - .25lbs of examiners measurement




Height-Standing


assist resident to stand on scale


•resident should be balanced and centered on scale with arms at their side


•raise folded measuring bar above residents head open and lower it gently until bar rests on top of their head not their hair


accurately record height within + or - .5 inch of examiners measurement

Skill 11 - Application of an Anti-Embolism Stocking (TED hose)

when asked by examiner explain what position the resident should be in when applying the stocking. Apply while resident is in bed or feet elevated


Hold foot and heel of stocking and gather up stocking turning stocking inside out down to the heel aids in application


smooth the stocking and over leg so hose is even snug and not twisted or wrinkled


• be sure that heel and toe are in proper location


the toe hole may be on top or bottom of toes depending on design
