1. MAR check- exp, calculation: total 2 meds _____
2. Record on PCS. *Show CE* 3. Id patient. ( name, dob, mr#) 4. Shake/ Roll NPH 5. Needle 6. Gloves, 7. wipe bottles 8. Air to cloudy *Show CE* (careful not to touch liq) 9. Air to clear *Show CE* 10. Invert bottle clear vertically and draw. *Show CE* 11. Invert bottle cloudy vertically and draw *Show CE* 12. Wipe area - circular motion 13. Pinch and inject 45 degree angle 14. discard needle in sharps container 15. DOCUMENT!!!!!!!! |

Place gauze pointing to area 12. Aspirate!!!! 13. inject slowly 90 degree angle 14. Gentle pressure with gauze 15. Place in sharps container 16. DOCUMENT!!!!!!!! |

GLOVES, CHECK SITE-edema, temp, take off gloves 5. 4 L's - affix label on bag, tube, hang, - lower primary level - make sure all clamps are locked - lower secondary to back flush 6. wash out old med if applicable, close clamp 7. insert new bag 8. open clamp 9. count drops per minute USING PRIMARY 10.CHECK EVERYTHING TUBEWISE **Perform hand hygiene **Verbalize the need to return to the patient’sroom as the piggyback infusion is ending to readjust the IV flow rate. 11. 2. Set up things in order of use ( tape, pen, ABD pad, gauze, 4x4, normal saline, sterile gloves, clean gloves, trashcan) 3. Clean gloves 4. Pull toward tape. 5. Assess site LTAK, COCA, 6. TAKE OFF GLOVES 7. DON STERILE GLOVES 8. wring out water from sponge 9. FAN OUT 10. Pack wound without touching surroundings 11. cover with pad 14. cover w ABD PAD 15. Tape 16. Date / initial |
