Cause of SOB #1
- Insufficient O2 to lungs |
obstruction (asthma, COPD, foreign body)
|
|
Cause of SOB #2
- Insufficient O2 diffusing from air to blood |
pulmonary oedema
pneumonia PE pulmonary fibrosis |
|
Cause of SOB #3
- Insufficient O2 getting transported |
Decreased cardiac output
Anaemia Shock |
|
Cause of SOB #4
- Increased respiratory drive |
Hyperventilation
Acidaemia |
|
Explain the paradoxes of the body's response to decreased cardiac output
|
1. Sympathetics increase the HR to compensate by stimulating Beta receptors H/E this increased cardiac O2 demand
2. Decreased CO = decreased renal perfusion = RAS system activation = fluid retention + overload |
|
Mx of cardiac failure
|
B-blockers to slow HR to allow for perfusion
Fluid overload = inhibit RAS by ACEi/ARB Underlying cause if often atherosclerosis therefore statins, aspirin and optimising other RF (DM, weight etc) |
