Cause of SOB #1
- Insufficient O2 to lungs
obstruction (asthma, COPD, foreign body)
Cause of SOB #2
- Insufficient O2 diffusing from air to blood
pulmonary oedema
pneumonia
PE
pulmonary fibrosis
Cause of SOB #3
- Insufficient O2 getting transported
Decreased cardiac output
Anaemia
Shock
Cause of SOB #4
- Increased respiratory drive
Hyperventilation
Acidaemia
Explain the paradoxes of the body's response to decreased cardiac output
1. Sympathetics increase the HR to compensate by stimulating Beta receptors H/E this increased cardiac O2 demand
2. Decreased CO = decreased renal perfusion = RAS system activation = fluid retention + overload
Mx of cardiac failure
B-blockers to slow HR to allow for perfusion
Fluid overload = inhibit RAS by ACEi/ARB
Underlying cause if often atherosclerosis therefore statins, aspirin and optimising other RF (DM, weight etc)
