what disease is caused by Shigella, and what are the symptoms?
Bacillary Dysentery
cramping, abdominal pain, diarrhea w/ blood and mucous
bacteria invade mucosal cells of distal ileum and colon, inflammation and ulceration occurs, but bact rarely penetrate wall or bloodstream
200> cause disease
What are the virulence factors of Shigella?
Heat labile exotoxin (shiga neurotoxin) *S. dysenteriae*
B subunit binds, A subunit inhibits protein synthesis and alters the epithelial cells' ability to transport electrolytes, leading to fluid accumulation in intestine. also neurotoxin can result in convulsions/coma
extrachomosomal "invasins" mediate intracellular spread of organism
What is shigellosis?
disease with asymptomatic carriers, resides in the gall bladder and GI tract.
symptoms: fever, cramps, watery->mucous->bloody diarrhea
spread by the 5 F's: fingers, flies, fomites, food, feces
S. dysenteriae is severe, S. sonnei is mild
Treat with fluids and ampicillin
How do you differentiate S. dystenteriae?
mannitol -
How do you differentiate S. flexneri?
Raffinose delayed
