PLISSIT
suggestions for easing sexual disfunction
P - Permission giving - normal to be in love with someone and find someone else attractive
L - Limited information (everyone has it, relax)
SS - specific suggestions (try new position)
IT - Intensive therapy (10% need sex therapy, PLSS doesn't work)
Masters and johnson approach to sex therapy
therapy in pairs, involved getting a surrogate partner, 4-5 hours per day for one week. counselors match gender of paitent.
Spectatoring
Performance anxiety in sex. sex becomes out of body critiquing experience. comparison to other men.
types of physical dysfunction
Neurological (spinal problems, no erection), vascular (decreased testosterone, decreased llibido, decreased estrogen, decreased lube), and vascular (pushing blood to pelvic region problems - vasocongestion, decrease erection, decrease vag lube)
Hypoactive sexual desire
low sex drive, 10-20% of me have a tumor of their pituitary gland, which means they screte prolactin decreasing sex desire. mostly cuased by psych issues.

types of sexual dysfunction in women
1) an/inorgasmia
2)lack of lubrication
3) vaginismus
4)dysparenuria
5)endometriosis
in/anorgasmia
primary- never have been able to reach orgasm (10%)
secondary - used to be able to but not currently
tertiary (coital) - 30-45% inability to orgasm from sex
quarternary (situational)
lack of lubrication cause
low estrogen

low estrogen. treat by checking hormonal levels, if normal = psychosocial
vaginismus
outer perivaginal muscles cease shut therefore sex is impossible. treatment is therapy and dilators to keep vaginal passage open
dysparenuria
pain during sex. could be from STI, but majority of this is psychosocial
endometriosis
lining of uterus breaks off into fallopian tube and then into abdominal cavity, sometimes encapsulates an ovary. causes pain during intercourse. treatment is BC pills, getting pregnant, and laser therapy (clip off layer)
sexual dysfunction types in men
2 types. premature ejaculation and erectile dysfunction.
premature ejaculation
inability to control when you ejaculate. treatment is:
1)masterbation before you get to "cliff"
2) masters and johnson grabbing base of penis to associate pain with sex
3)moore and kotrla - blamed women. one partner has the "lets get this overwith attitude", or theres too mcuh foreplay or anxiety within female is felt by the man.
primary ED
may be able to get erection, but not able to maintain during sex to penetrate. usually cuased by physical/vascular problem but could be psychosocial
secondary ED
man was fine before but is now having problem. cause is usually an accident or bike damage cuasing decreased blood in area.
treatments of ED
1) papaverine - injection to chemical induce erection
2)alprostadil - pellet put into eye of penis
3) cialis/viagra - orignally for hypertension treatment
4)penile prosthetics - (semi rigid silicon rods, pneumatic penis pump, vaccuum pump)
