Anteriorly,neck extends from
Lower border of mandible to manubrium.
Posteriorly,necks extends from
Superior nucheal line on occipital bone down to C7-T1 disc.
Superficial fascia of neck is special for 2 reasons.Name them.
-Anteriorly,it contains a thin sheet of muscle-platysma which runs up to blend with muscles of face(supplied by facial nerve-cervical branch)
-has important veins anteriorly--> External jugular + Anterior jugular
Ant. Jugular vein empty into
External Jugular vein
External Jugular vein
-more laterally
-starts at angle of jaw & runs down vertically on SCM
-drains lots of superficial structures.
Ant. jugular vein is
On either side of midline
Trachea is always open due to
U-shaped cartilage rings
Neck is divided into
4 longitudinal compartments(each contained in a sleeve of fascia)

Surrounding visceral cpt is
Pre-tracheal fascia
Pre-tracheal fascia is divided into :
-Pre tracheal fascia ANTERIORLY
-buccopharyngeal fascia POSTERIORLY
WHat surrounds vertebral cpt?
-Prevertebral fascia
All layers of fascia have
Spaces in between-->any infections which get into spaces might spread into spaces down to thorax.
Carotid sheath
-Carotid artery MEDIALLY
-Int. Jugular vein
-Vagus nerve
Into which 2 main regions is neck div?
-Anterior triangle( infront of SCM ,below inferior margin of mandible)
-Posterior triangle(behind SCM +infront of trapezium + above middle 1/3 of clavicle
Carotid artery bifurcates at
C3/C4(same level of thyroid cartilage)
Carotid a.
branch of aorta
Internal carotid a. has
NO branches in neck.(however contains carotid sinus & carotid body)
External carotid artery
-ascends in ant. triangle
-heads towards parotid gland
-ascending,in front of ICA
-give 6 branches during ascent
What are the 3 branches of ECA that are in front?
-Superior thyroid artery
-Lingual artery(floor of mouth + tongue)
-Facial artery(winds over inf. border of mandible)
What are 2 branches of ECA at the back?
-Post. auricular artery(behind ear)
-Occipital artery(behind skull)
What is the medial branch of ECA ?
Ascending pharyngeal artery(ascends with deep structures of pharynx)
Structures that come out of jugular foramen or hypoglosseal canal come
Straight at top of ANTERIOR triangle.(CN 9,10,11 + 12)
IJV
-Lateral in carotid sheath
-has all tributaries that corresponds to branches of ECA
Vagus nerve
-descends with IJV and carotid system, right into thorax.
Somatic sensory fibres of CN 10
Mucous membranes of larynx
Branchial motor fibres of vagus nerve
-Muscles of pharynx,larynx,soft palate
Accessory nerve
Goes backward into POSTERIOR triangle-->crosses post. triangle and supplies SCM and trapezius
Test for CN 11
Shrug shoulders
Hypoglossal nerve
-Motor supply to tongue,except palatoglossus
-if damaged, tongue deviated to affected side
-passes between carotid artery & jugular vein.
Thyroid gland
-2 lobes between upper border of thyroid cartilage above + 6th tracheal ring below
-isthmus between 2nd and 3rd tracheal rings
-enclosed by pretracheal fascia
-part of visceral cpt
Thyroid moves up and down on swallowing because
pretracheal fascia moves up and down with swallowing
If there is lump in neck which does not move up and down on swallowing,
lump is NOT inside pretracheal fascia.
Isthmus stuck to
Tracheal rings 2,3,4.
Pre tracheal fascia extends to
Hyoid bone(attaches to it)
Thyroid gland is found
-deep to strap muscles
-lateral to trachea/oesophagus
-overlap carotid sheath
What happens in 50 % of cases with thyroid gland?
-Extrapyramidal lobe that ascends from isthmus to left(pyramidal lobe)--> remnant of hypoglossal duct
Thyroid is
Outgrowth of embryonic pharynx
Embryonic pharynx
migrates to front of trachea
Path of migration marked
by thyroglossal duct
Recurrent laryngeal nerve sits in
Groove between trachea and oesophagus
R.L.N embedded in
-Fascia of lobes of thyroid gland from behind.
Site of dev. of thyroid gland
Foramen caecum
Enlargement of thyroid gland
-Compress trachea & oesophagus--> difficulty swallowing
-invade R.L.N on one side--> hoarse voice
-compress IJV--> engorged veins above pt of obstruction
Thyroid gland
Most vascular gland in body
Parathyroid glands(peasize)
-2 pairs on posterior aspect of each lobe of thyroid gland--> 2 superior and 2 inferior
Blood supply of thyroid
-Paired superior thyroid a.(1st branch of ECA)
-paired inferior thyroid a.(branch of subclavian a)
-branches anastomose vertically and across midline
Occasionally,in 10 % of cases,
Single thyroid a. passes along isthmus--IMA artery
Veins of thryoid gland
-3 pairs of veins(superior, middle and inferior thyroid )
Cervical plexus
from ventral rami of C2,C3,C4(under SCM)
-superficial(cutaneous) branches to skin
-Deep branches
Deep branches of cervical plexus called
Ansa cervicalis(motor to strap muscles of neck-attach to hyoid)
Rami of brachial plexus
Pass between scalenus medius and anterior
Phrenic nerve
C3,C4,C5
Subclavian system supplies
-upper limb
-almost 1/2 brain
-most of spinal cord(vertebral a)
-part of thoracic wall
Brachial plexus travels along with
Axillary artery
Main branches of subclavian system in neck
-Vertebral(normally 1st branch)--> passes in triangle between longus colli & scalenus anterior.
-Thyrocervical trunk(gives off inferior thyroid a. to thyroid gland)
Superficial nodes drain into
Deep nodes
Name the 2 groups of Superficial cervical nodes.
-Horizontal(junction of head & neck)
-Vertical(along external jugular vein)
Horizontal superficial cervical nodes consist of
-Submental
-Submandibular
-Parotid
-Post. auricular
-Occipital
Deep cervical is along
Int. Jugular vein
Deep cervical is made up of
-Upper group(ANT triangle)
-Lower group(POST. triangle)
Upper group consists of
Jugulo-digastric(JD) or tonsillar node --> drains palatine tonsils.
Lower group consists of
Supraclavicular nodes-final sentinel node.(JO)
Name the 3 deep muscles of neck
-Levator scapuli
