Assess client's level of disorientation, obtain a drug history, obtain urine sample for lab, keep client in a quiet enviornment, private room, observe client frequently, one to one supervision in worst cases, accompany client while ambulating, pad headboard and side rails, remove harmful materials, follow meds as ordered, correct misconception about substance abuse, do not allow client to rationalize or project behavior, encourage group activity, nutrition, sodium may need to be restricted.
