Scale (exfoliation)
excessive shedding of the skin (the outer layer of skin is composed of fundamental elements called keratin

Ex: thin scaly (keratotic patch) on the R nasal dorsum
Crust (scab)
dried serum, pus or blood. They can be characterized as friable (easily removed and the base would likely bleed) vs. soft vs. superficial vs. impetiginous (means impetigo-like referring to honey colored crust)
Excoriations (abrasions)

Ex: excoriated lichenified (thickened such that the skin has become firm and heaped up) eczematous plaques on the upper back
Fissures
linear cracks

Ex: thin eczematous plaques on the BIL hands with fissuring on several fingertips
Erosions
loss of the epidermis. Heals w/o a scar

Ex: central erosion surrounded by and erythematous plaque

complete loss of the epidermis and extending into the dermis or subcutaneous fat

Ex: oval full-thickness ulcerations of the R parietal scalp
