What is scoliosis?
It is a skeletal deformity which there is a lateral curvature of the spine that causes changes in the spine, chest, and hips.
What is affected when there is severe curvature of the spine (scoliosis)?
cardiopulmonary (respiratory compromise) and neurological function and result in negative self-esteem.
When does Scoliosis occur?
it can occur at any age but is more often seen in adolescent females.
What characteristics would a nurse find with a patient with idiopathic scoliosis?
The patient will have unequal hip height and shoulder height, scapular and rib prominence, and a posterior rib hump that is visible when the child bends forward at the waist.
What diagnostic test confirms scoliosis?
Radiographs (x-rays)
At what age do children need routine screening?
10-15
What does routine screening involve?
Involves posterior observation of the undressed child for curvature, asymmetry, and rib hump.
What position is the patient in while recieving a routine screening?
In a bending forward at the waist with arms and head hanging downward position.
Curvatures of less than ___ degrees require no treatment.
20 degrees.
Braces are used for what type of curvatures?
Moderate curvatures
What is the purpose for braces with scoliosis?
It slows the progression of scoliosis until the spine is mature.
What are the two types of braces for scoliosis?
Boston brace and (TLSO) Thoracolumbosacral orthotic brace.
When is surgical intervention indicated?
Indicated in severe scoliosis or scoliosis that does no respond to bracing.
What does surgery consist of?
Consists of spinal fusion and the inserton of a stabilizing rod such as a Harington rod or L rod.
What type of nursing diagnosis would be given for a pt. with scoliosis?
body image disturbance r/t scoliosis or impaired imobility r/t scoliosis.
How long does the brace need to be on?
For 23 hrs. and only taken off during bathing.
What is the prognosis?
Depends on the severity of the condition and compliance with tx.
How should the brace be worn?
brace should be word over the t-shirt to decrease skin irritaion and another loose clothing over the brace that camouflages the brace.
What is needed before the pt. goes into surgery?
An informed consent, kept NPO, v/s every 4 hrs., ask for allergies to iodine and shellfish.
What is needed to be done for post-op care?
Assess neuro status, (CMS) circulatory motor and sensitivity, and 7p's.
When are pain meds. given after surgery?
Around the clock (q 2)
Log roll is done for how many days and with how many nurses?
It is done for atleast 5 days with three nurses.
Why is the patient on NGT after surgery?
To prevent aspiration and decompress the stomach.
Why is it important to encourage the pt to do their ADL on their own?
To increase self-esteem. (assist with ambulation under fall precaution).
