The symptoms of withdrawal from a narcotic drug depend on how severe your addiction is. On a scale of 0 to 4, the symptoms are:

0: anxiety and strong desire for the drug

1: watery eyes, watery discharge from the nose, and yawning

2: above symptoms plus dilated (enlarged) pupils of the eyes, loss of appetite, shakes, hot and cold flashes, and aching of your whole body

3: severe shakes, hot and cold flashes, aching, fever, high blood pressure, fast pulse, and rapid breathing

4: diarrhea, vomiting, low blood pressure, and dehydration.

Additional symptoms of withdrawal from severe addiction may include:

hepatic encephalopathy
With severe liver impairment, toxic substances normally removed by the liver accumulate in the blood and impair the function of brain cells. If there is also portal hypertension, and subsequent bypassing of the liver filtration system of blood flowing in from the intestines, these toxic substances can travel directly to the brain, without being modified or purified. Signs can include impaired cognition, a flapping tremor (asterixis), and a decreased level of consciousness including coma (hepatic coma or coma hepaticum), cerebral edema, and, ultimately, death.

LITHIUM – S&S OF TOXICITY
• Early Signs of toxicity: Thirst!!!!, confused, N&V, diarrhea, , tremors, increased Bp , ireg H.beat
LITHIUM
Maintenance level: 0.4-1.3
• Levels should not exceed 1.5
• Lithium needs to be tapered! Do not stop abruptly !
• If Lithium ↓ check NA----lithium decreases sodium reabsorption in the kidneys and could lead to deficiency in NA
• A low NA level can lead to an increase in Lithium retention and produce toxicity.
• Take with meals!
• If signs of toxicity: you can say don’t take the next dose but make an immediate appointment to get the dosage changed – dr. may order a lavage - don’t just stop abruptly.
• ALWAYS ASK “ARE YOU TAKING IT AS PRESCRIBED?”

• Risperidone is an antipsychotic medication. It works by changing the effects of chemicals in the brain.
• Risperidone is used to treat schizophrenia and symptoms of bipolar disorder (manic depression). Risperidone is also used in autistic children to treat symptoms of irritability.
Wernicke’s encephalopathy
o Wernicke encephalopathy is a serious disorder caused by thiamine (vitamin B-1) deficiency
o Wernicke's encephalopathy involves damage to multiple nerves in both the central nervous system (brain and spinal cord) and the peripheral nervous system (the rest of the body).
o The cause is generally attributed to malnutrition, especially lack of vitamin B1 (thiamine), which is common in those with alcoholism.
o Heavy alcohol use interferes with the break down of thiamine in the body, so even if someone with alcoholism follows a well-balanced diet, most of the thiamine is not absorbed
o Korsakoff syndrome, or Korsakoff psychosis, tends to develop as Wernicke's symptoms do away. This involves damage to areas of the brain involved with memory. Patients often attempt to hide their poor memory by creating detailed, believable stories about experiences or situations. This is not usually a deliberate attempt to deceive because the patient often believes what he is saying to be true.
