Factors influencing patient safety:

Developmental stage, individual lifestyle, mobility, sensory/communication impairment, and lack of safety awareness (could be due to cognitive issue)

Four categories or risk in health care agency:

Falls, patient inherent accidents, procedure related accidents, equipment related accidents.

Patient inherent accidents:

Behavior or characteristics of patient.

Procedure related accidents:

Have to do with surgery or placing an IV, something went wrong because we were not paying attention to safety.

Equipment related accidents:

Ignored an alarm, sometimes its malfunction, sometimes equipment gets piled up in hallways, etc.

Fall reduction interventions:

Hourly rounding, color coded socks, signs, arm bands, nonslip socks, call lights, mobility aids.

5p's of hourly rounding are:

Pain, potty, position, possessions, plan

What are the two categories of restraints?

Chemical or physical

Risks with using restraints

Pressure ulcers, pneumonia, constipation, incontinence, restricted breathing or circulation.

Psychosocial risks with restraints:

Humiliation and agitation

What is required for restraints?

Physician order, alternatives tried first for instance distractions. Patient must have tried to prevent therapy such has pulling out IV. Or causing harm to self or others.
