Patient's perception of general state of health at present, difference from usual state, vitality and energy levels, body odors, fever, chills and right sweats


Ask the patient to describe his general health

General

Inquire about alopecia, excessive growth of hair in unusual locations (hirsutism), use of chemicals on hair, dandruff, pediculosis, scalp lesions

Hair

Ask about change in nails such as splitting, breaking, thickened, texture change, onchomycosis

Nails

Ask about any experience of blurring of vision


Ask about visual acuity (use of eye glasses, contact lenses)


Ask about photophobia, excessive tearing, night blindness, diplopia, bloodshot eyes, pain, blind spots, flashing lights, halos around objects, floaters


Ask about any diagnosis of cataract, glaucoma or other eye problems


Loquire about use of sunglasses or any other protective eyewear

Eyes

Ask about the patient's method of cleaning his ears


Ask about hearing deficits, use of hearing aids, ear pain, phonophobia, discharges, vertigo, ringing in the ears (tinnitus), usual noise level, previous car infections, use of ear protection

Ears

Explore patient's dental habits (brushing, flossing, mouth rinses)


Ask about experiences of toothache, tooth abscesses, guin bleeding, difficulty chewing, sore tongue, change in taste, change in salivation

Mouth

Ask about changes in voice (hoarseness), experiences of sore throats, dysphagia, pain or stiffness of the neck


Inquire about possible thyroid problems, previous episodes of lymphadenopathy


Ask about possible experiences of tonsillectomy, adenoidectomy

Throat and Neck

Ask about experiences of pain or tenderness on breasts


Inquire about the presence of lumps or discharges, change in size or occurrence of dimpling along the breasts


Ask about previous diagnosis of benign breast disease or breast cancer

Breast and Axilla

Ask about previous experiences with respiratory problems such ashortness of breath, hemoptysis, cough,


Ask about previous diagnosis of pneumonia, emphysema, asthma, tuberculosis, frequent respiratory infections


Ask about previous exposure to TB


Ask about last chest x-ray or PPD and their results if applicable

Respiratory

Ask about experiences of paroxysmal nocturnal dyspnea, chest palpitations, edema, pains, palpitations, syncope, orthopnea, edema, cold or discoloured hands or felt, leg cramps, intermittent claudication


Ask about previous diagnosis of myocardial infarction, hypertension, valvular disease, thrombophlebitis, deep vein thrombosis


Inquire about last ECG and result if applicable

Ask about previous dagnoses of kidney stones, urinary tract infections

Explore possible experiences of weakness, bone deformities, broken bones, dislocations, sprains, crepitus

Ask about present weight, usual weight, desired weight, food intolerances, food likes end dislikes, caffeine intake, dietary habits

Nutrition

Look for or ask about exophthalmos


Ask about experiences of fatigue, change in size of head, hands or feet


Inquire about weight changes, heat and/or cold intolerances, excessive sweating, polydipsia, ployphagia, polyuria, changes in body hair distribution

Endocrine

Ask about any notes on lymph node enlargement or tenderness
