Child w/ developmental disability

-late to appts, and frequently no show

-not doing HEP

-quickly regressing

-talk w/ parents & no longer come to PT

What to do

Report family for neglect (not child abuse)

-witholding necessary medical care

Discharge bc parents stoped bringing child

child abuse

physical, sexual, oe mental insult

Biceps brachii decreased strength

-chronically inflamed

-painful arc of motion

TX

active eccentric contractions in pain free range

-for chronically inflamed muscle focus should be on eccentric contractions bc there is less effort and stress placed on contractile unit compared to concentric contractions at same level of work

-isokinetic, isometric, isotonic do not allow for pain free mm contraction & can further inflame the mm

US

-pt feels strong ache

Why

decrease intensity

-acoustical energy is reflected from the bone into the bone-tissue interface, resulting in rapid temp elevation & stimulation of highly sensitive periosteum of the bone

US

-why increase Tx area

minimize ability to elevate tissue temp & would not get strog ache from rapid tissue temp elevation

US

-adding transmission medium

-encourages transmission of accoustical energy and more rapidly elevate tissue temp

US

-frequency

has to do with depth of penetration of US energy

-not rate/speed tissue temp is elevated

TYPE II SLAP repair 1 week ago

TX

careful ROM of internal rotators

-does not create stress to repair & create peel back mechanism that ER does

TYPE II SLAP lesions characterized by

-detachment of the superior labrum & the origin of the tendon of the long head of biceps that results in instability of biceps-labral anchor

-surgery re-attaches labrum & biceps anchor

-contraction & stretching of the biceps should be avoided during max protection phase

-NO ER ROM or stretching

deep partial thickness burns over post thigh & calf that are healed

-TTP & swelling & pain on movement in the hip area

-palpation reveals a mass

What to do

report findings to physician

-these are signs & sx's of heterotrophic ossification (abnormal bone growth around a jt which can lead to serious ROM limitations)

NO: massage, aggressive ROM
