ACE Inhibitors
-pril
Captopril; only ACE-I with a sulfyl moiety
All pro-drugs except Captopril and Lisinopril
Long t1/2; except Captopril (~2hrs)
ARBs
-sartan
Only two pro-drugs (esters); Candesartan and Olmesartan -> activated in intestinal epithelium
ACE/ARB Side effects
Dry cough; due to increase in Bradykinin, less common with ARBs
Angiodema; due to increase in Bradykinin, common in face, tounge, lips and throat (can cause airway constriction). Less common with ARBs
Hyperkalemia; contraindicated in patients taking K+ supplements or those on K+ sparing diuretics
-pair with Thiazide to offset increase in K+
ACE/ARB Contraindications
Bilateral renal artery stenosis
Prior angiodema
Pregnancy
