infant shows signs of cyanosis,d yspnea,and right ventricular failure,but is well-developed and well-nourished; examination reveals systolic ejection murmur heard best during expiration, split 2nd heart sound. (A critical pulmonic stenosis indicates right-to-left shunt through a patent foramen ovale, cyanosis markedly decreased right ventricular compliance, and right ventricular failure.)

MOST LIKELY DIAGNOSIS: pulmonic stenosis.

CHEST X-RAY: diminished pulmonary circulation, cardiomegaly. EKG: right ventricular hypertrophy. DIAGNOSIS CONFIRMED BY: echocardiogram.
TREATMENT:
(a)
Mild and asymptomatic patient can lead normal life,no surgery necessary.
(b)
Moderate-to-severe with symptomatic patient:

TREATMENT OF CHOICE: balloon pulmonary valvuloplasty is initial treatment of choice. Newborn with critical
stenosis needs balloon valvuloplasty or surgical valvotomy as soon as possible.
