General: Best diagnostic clue: Ill-defined hypointense vertebral marrow on T1WI with loss of endplate definition on both sides of disc with two adjacent vertebrae with intervening disc with loss of disc height, signal abnormal, destruction of endplate. Can have paraspinal/epidural extension (abscess/phlegmon)
Xray: Negative early, Endplate osteolysis and sclerosis. Paraspinal ST density. Fusion across disc late in disease CT: Endplate osteolytic/osteosclerotic changes. +/- ST gass. E+ - enhancing disc, marrow, paravert ST MRI: Disc: Low T1, High T2, enhancement, height loss. VB: Low t1, high t2, E+ with gado. Paraspinal/epidural phlegmon t2 bright, enhancing, cord comp Protocol advice: T2fat sat (marrow edema), T1 pre/post (2 planes for each sequence), Coronal T1post |

General Features:
Discs usually okay, paraspinal abscess. Affects anterior VB, posterior elements. Multiple non-contiguous vertebrae. XRay: Irregular endplate, osteolysis, slow to see findings CT: endplate, fragmented VB. Calcified paravertebral abscess with enhancement MRI: T1 low marrow, low signal abscess. T2 high signal marrow,disc,abscess. Post Gad marrow diffuse enhancement. Disc usually spared |
