From cave-paintings, remains, studying tribes/ people who were isolated from society who lived like them e.g. Australian aborigines

Child birth, infections, warfare, famine

Sometimes they survived as there have been skulls found with regrowth, No - they usually died as they would hit the brain, blood loss or pain. It wouldn't have helped relieve the pain.
