methylergonovine (Methergine) p 843
Classification: oxytocic
Indications: prevention and tx of postpartum or post abortion hemorrhage
Action: directly stimulates uterine and vascular smooth muscle. Therapeutic effects: uterine contraction
Distribution: enters breast milk in sm quantities
Metabolism: liver
Half life: 30-120 min
Contradictions: hypersensitivity: OB- should not be used to induce labor, extreme caution in 3rd stage of labor.
Adverse reaction: N/V, cramps
Route/Dosage: PO (Adults) 200-400mcg (0.4-0.6 mg) q6-12 hr for 2-7 days. IM,IV (Adults) 200mcg (0.2 )q2-4 hr for up to 5 doses
Nursing implications: monitor BP, HR and uterine response
ibuprofen (Motrin) p 678
Classification: antipyretic, antirheumatic, nonopiod analgesic, non-steroidal anti-inflammatory agent
Indications: tx of mild/moderate pain, fever. tx of inflammatory d/o including RA and OA, dysmenorrhea
Action: inhibits prostaglandin synthesis. Therapeutic effects: <pain &inflammation, < fever
Distribution: does not enter breast milk
Metabolism: liver
Half-life: children 1-2hr; adults 2-4hr
Contradictions: hypersensitivity; active GI bleeding or ulcer disease; OB: avoid after 3 wk gestation
Adverse reaction: H/A, N/V, constipation, & dyspepsia
Route/Dosage: PO (adults) anti-inflammatory 400-800mg q6 PRN (not to exceed 3200mg/day/ analgesic 200-400mg q4-6 hr (not to exceed1200 mg/day); PO (children 6mo-12yr) anti-inflammatory 30-50mg/kg/day (max dose: 2.4g/day) antipyretic 5-10mg/kg (max dose 40mg/kg/day; PO (infants) analgesic 4-10mg/kg/dose q6-8 hr
Nursing implications: assess for hypersensitivity; S/S of skin rash/GI bleeding, assess pain and monitor temp
MMR-Measles, Mumps, and Rubella vaccine
Routine pediatric immunization
Route/Dosage: 0.5 mL subcut at 12-15 mo and at 4-6 yr
Contraindications: allergy to egg, gelatin, or neomycin; active infection, severe immunosuppression; pregnancy
Adverse reaction: burning, stinging, pain at injection site; arthritis/arthralgia; fever; encephalitis; allergic reactions
MMR- measles, mumps, ad rubella vaccine
routine adult immunization
Indications: adults born in or after 1957 w unreliable hx of MMR illness; high risk groups (HCP, travelers, college students); women of child bearing age (w o evidence of rubella immunity)
Dosage/Route: 1 or doses of 0.5 mL subcut (w >/= 28 days between doses); high risk groups should receive a total of 2 doses given 1 mo apart.
Contraindications: avoid becoming pregnant for 4 wk after immunization
Adverse reaction: Immunosuppression may < antibody response to injection and > the risk of viral transmission
