What is PMR? (3)
Syndrome: TRIAD
1. Muscle pain
2. Stiffness
3. Increased ESR >40mm/hr
Associated features (1)
Giant cell arteritis in 15-20%(although PMR is not a true vasculitis)
Who is affected (2)
Women (3:1)

Over 70 yrs
Symptoms (5)
Proximal muscle stiffness e.g. neck, upper arms, buttocks, thighs.

Rapid onset pain (tender on palpation)

Early morning stiffness

Night pain

Weight loss, fatgue, depression, night sweats.

NB: THERE IS NO MUSCLE WASTING, if there is a neuro disease is more likely.
Investigate
ESR (if low check CRP which may be elevated)

Normochromic normocytic anaemia
Rx:
Corticosteroids

Prednisolone

(should be much better in 72hrs, resolved by 7days, if it hasn't the diagnosis is wrong)

Sparing agents - methotrexate + azathioprine

Prophylaxis bone degen - bisphosphonate + calcium supplements
