What is PMR? (3)

Syndrome: TRIAD
1. Muscle pain 2. Stiffness 3. Increased ESR >40mm/hr |

Associated features (1)

Giant cell arteritis in 15-20%(although PMR is not a true vasculitis)

Who is affected (2)

Women (3:1)
Over 70 yrs |

Symptoms (5)
|
Proximal muscle stiffness e.g. neck, upper arms, buttocks, thighs.
Rapid onset pain (tender on palpation) Early morning stiffness Night pain Weight loss, fatgue, depression, night sweats. NB: THERE IS NO MUSCLE WASTING, if there is a neuro disease is more likely. |

Investigate

ESR (if low check CRP which may be elevated)
Normochromic normocytic anaemia |

Rx:
|
Corticosteroids
Prednisolone (should be much better in 72hrs, resolved by 7days, if it hasn't the diagnosis is wrong) Sparing agents - methotrexate + azathioprine Prophylaxis bone degen - bisphosphonate + calcium supplements |
