What are the Centor criteria?
They are criteria to predict the presence of group a strep. (F)ever, (A)denopathy, (C)ough, (E)xudates (younger than 15 add 1 point, older than 44, subtract one point)
How are the centor criteria used?
3 or more criteria has a positive predictive value of 40-60% (risk 60%). 1 or less criteria has a negative predictive value of 80% (risk less than 10%).
How is the rapid strep test affected by the centor criteria?
The more criteria fulfilled, the more sensitive and specific the rapid strep antigen test.
What percentage of pharyngitis is strep? What is the precentage of viruses? what percentage is no pathogen found? what percentage are non-strep bacterial causes?
15% of pharyngitis is strep. 50% of pharyngitis is viral. no pathogen is found in 30%. Non-strep bacteria cause less than 5% of strep.
What are the reasons to pursue antibiotic therapy in strep pharygitis?
Prevent rheumatic sequelae, prevent suppurative sequelae, reduce symptoms, prevent transmission.
What criteria must be met to prescribe antibiotics for pharyngitis?
Greater than 2 of 4 Centor criteria met, positive rapid strep
