the sender (sources of drugs), the receiver (recipient of drugs), and the DEA
What happens with the destruction of controlled substances?
you must contact the nearest DEA office, yearly destruction is allowed,complete form 41, letter requesting permission is required: date, names, of 2 witnesses, method, 2 weeks prior to destruction
What is DEA form 41 used for?
form of destruction, pharmacy must keep a copy and send one to DEA
What is the exception to DEA authorization for destruction?
prior DEA authorization is not required if an authorized member of a state law enforcement authority or regulatory agency witnesses he destruction

What are the security requirements for pharmacies in regards to controlled substances?
federal
must keep schedule II, III, IV and V CS ia locked cabinet or dispersed throughout the non-controlled inventory to deter theft
What is form 106 used for?
report of theft or loss of controlled substances
when must significant loss of controlled substances be reported to DEA?
immediately! via phone, fax or written message
notify local police and complete form 106
What info is required on form 106?
name and address of pharmacy, DEA registration number of pharmacy, date of theft, name and number of police notified, type of theft, listing of symbols rcost code used by pharmacy on containers, list of CS missing from inventory
