Methotrexate (Trexall, Rheumatrex)

Class: DMARDs-Non Biologic; Musculoskeletal Antirheumatics

MOA: Inhibits cells in S phase by inhibiting dihydrofolate reductase; inhibits T cell proliferation, reduce B & T cell activity

Uses: Tx cancer, severe rheumatoid arthritis

SEs: Hepatic fibrosis, GI ulceration, bone marrow suppression
Sulfasalazine (Azulfidine)

Class, MOA, USes, SEs?
Sulfasalazine (Azulfidine)

SEs: Pregnancy category B, N/D/V, anorexia, headache, rash, GI and dermatological effects
Leflunomide (Arava)

Class, MOA, USes, SEs?
What is unique about its metabolite?
Leflunomide (Arava)

Class: DMARDs-Non Biologic; Musculoskeletal Antirheumatics

MOA: Inhibits pyrimidine synthesis and limits lymphocyte proliferation and reduces inflammation

Uses: Rheumatoid Arthritis

SEs: Hepatotoxic, potent suppression of immune system increase risk of infection, pregnancy category X

Other: Active metabolite is responsible for anti-inflammatory effects
Hydroxycholorquine (Plaquenil)

Class, MOA, USes, SEs?
What is important about it's duration?
Hydroxycholorquine (Plaquenil)

Class: DMARDs-Non Biologic; Musculoskeletal Antirheumatics

Uses: Tx of severe rheumatoid arthritis in conjunction with other drugs; Antimalarial agent

SEs: Retinal Damage

Duration: longer onset of action (3-6 months)
Entanercept (Enbrel)
Infliximab (Remicade)
Adalimumab (Humira)

Class, MOA, USes, SEs? Routes?
Entanercept (Enbrel)

Class: DMARDs-Biologic TNF-a Antagonist; Musculoskeletal Antirheumatics

MOA: Prevents TNF-a (tumor necrosis factor- inflammation modulator) from interacting with its receptor

Uses: Decrease progression of rheumatoid arthritis that is moderate to severe; Tx psoriatic arthritis, active ankylosing spondylitis, and crohn’s disease

SEs: erious infections (TB and fungal infections), upper respiratory tract infections, rhinitis

Route: Entanercept (Enbrel) and Adalimumab (Humira) are IM
Infliximab (Remicade) is SubQ
Rituximab (Rituxan)

Class, MOA, Uses, SE?
Rituximab (Rituxan)

Class: DMARDs-Biologic B-lymphocyte depleting agents; Musculoskeletal Antirheumatics

MOA: Forms a complex on the surface of B-lymphocytes, targeting them for destruction by the immune system

Uses: Moderate to severe Active Rheumatoid arthritis

SEs: Progressive multifocal leukoencephalopathy, bronchospasm, arrhythmias, hypotension, mucocutaneous skin reactions, hyperglycemia, anemia, back pain, anaphylaxis, angioedema
Abatacept (Orencia)

Class, MOA, Uses, SE?
Abatacept (Orencia)

Class: DMARDs-Biologic T-cell activation inhibitors; Musculoskeletal Antirheumatics

MOA: Prevents antigen presenting cells from fully activating T-lymphocytes

Uses: Reduce S/S of moderate to severe active rheumatoid arthritis in adults

SEs: Increased risk of infection
