What are the 3 ACE inhibitors discussed in class?
1. Captopril
2. Enalapril maleate
3. Benazepril
What is the primary MOA of ACE inhibitors?
Inhibiting angiotensin I to angiotensin II
What stimulates renin release from kidney JG cells?
Reduced renal perfusion (i.e. decreased BP)
What does Renin stimulate the formation of?
Stimulates formation of angiotensin I, which is then metabolized by ACE (in lungs) to angiotensin II
What is the effect of Angiotensin II?
vasoconstriction
What does Angiotensin to stimulate?
Stimulates release of aldosterone
What effect does Aldosterone have in the kidneys?
Increased Na+ and water retention at kidney
What are the 2 major effects of ACE inhibitors?
• Cause peripheral vasodilation
• Facilitate Na+ excretion and diuresis
What are the cardiovascular effects of ACE inhibitors?
* Decreased pulmonary capillary and arterial pressure

* Decreased HR, decreased BP
What are ACE inhibitors commonly used for?
treatment of classes of heart failure
What are some adverse effects of ACE inhibitors?
* Should monitor renal function during administration to ensure adequate perfusion of kidneys

* May see sporadic cases of azotemia

* Should limit concurrent use of diuretics and NSAIDs

* May see coughing and angioedema
What are some drug interactions to keep in mind when giving an ACE inhibitor?
* ACE inhibitors potentiate effect of diuretics and should be co-administered with great caution

* ACE inhibitors given with NSAIDs will have diminished effect
Where is Enalapril metabolized?
Kidney
Where are Benazepril and Ramipril metabolized?
both liver and kidney
