Pharmacogenitics/genomics
What is the difference between pharmacogenetics and pharmacogenomics?
Pharmacogenetics - single gene
Pharmacogenomics - genome many genes
Pharmacogenetics/genomics
What inherited deficeiency is associated with fetal hemolysis following exposure to fava beans antimalarials such as-quine, infections or moth balls? What does the CDC advise should be done before any prescription for primaquine or quininesulfate is given?
G6P DH.
Tests for deficiency before prescription.
Pharmacogenitics/genomics
What type of cancer and what drug used to treat that cancer are almost universally preceded by genetic testing prior to administration? What gene is tested? This testing is relevant to what other 2 drugs?
Acute lymphocidic anemia which is degraded by TPMT (without this gene can cause anemia).
Azathioprine and Thioguanine
Pharmacogenetics/genomics
What drug is the most dose adjusted drug? For what drug is there notably precise dosing information given genetic and environmental info? What 4 genes have polymorphisms that are included in the algorithm by which the predicted maintenance dose is obtained? What is the physiological relevance of each to the dosing of the drug?
Most adjusted- Warfarin, Coumadin
4 genes- VKORC-1 (Drug target for warfarin, gets the drug to work). CYP4F2 (Vitamin K/E oxidase). GGCX (key component of vitamin K cycle). CYP2C9 (Clears warfarin).
Pharmacogenetics/genomics
What is the state of the precision of dosing information for the vast majority of other drugs, even those with good info concerning how pharmacogenomics interacts with their pharmacogenetics?
Not many drugs are able to be calculated in that way
Pharmacogenetics/genomics
For an individual with the HLA-B5701 allele present (allele of the human leukocyte antigen) the dosing info is easy. What is the drug and what is the advice and what would happen were the advice not followed? What is the disadvantage of following the advice?
Drug- Abacavir (HIV drug).
HLA-B5701 can cause extreme rash allergic rxn. If gene is present dont give the drug. Disadvantage is you cant take a drug proven to help if you have the gene.
Pharmacogenetics/genomics
How useful is a persons apparent genetic heritage as a biomarker to serve as a surrogate for gene sequence information?
Beta blockers have greater effects in whites than blacks because of mutation in GRK gene. Therefore blacks are harder to treat with beta blockers. Individual
Pharmacogenetics/genomics
Why was BiDil, a combination of two old drugs recently able to get patent protection? What classes of drugs make up BiDil?
BiDil is two vasodilators added together having a greater effect on blacks. The targeted group allowed patent protection.
Pharmacogenetics/genomics
How was the cancer drug Iressa also called (geftinib) able to gain approval for use in non small cell lung cancer despite initial evidence that it was not statistically significantly different than placebo?
Targeted at Japanese non smokers with lung cancer. A very narrow/specific target. Effective only in this target population.
