What are the Thyroid hormones?
T3=Triiodothyronine
T4=Tetraiodothyronine |
Physiologic effects of Thyroid Hormones?
-increased CO
-increased HR -increased RR -increased BMR -Brain Development -Thickens Endometrium |
Which one is given for Thryoid replacement?
T4
Hyperthryroidism?
-Grave's Disease
-may be subclinical (low TSH/normal T3-T4) |
What is overt hyperthroidism?
-Low TSH
-High free T3 and T4 |
What is the most common cause of hyperthyroidism?
-Grave's disease
-Autoimmune (antibodies attack the thyroid gland and cause a goiter and overprod. of thryoid hormone). |
TX of Grave's Disease?
1) reduce the amt of functional thyroid tissue
2) inhibit thryoid homrone secretion/production 3) Adjunctive tx(control sxms) |
Poylthiouracil (PTU)
-Older drug
-A thioanamide compound which inhibits biosynthesis of thyroid hormone -Inhibits T4-T3 conversion -Use T4/T3/TSH as levels as guides for taper |
PTU dosing
6 weeks tx for a eurothyroid condtion
-long term treat 1-2years -max dose 1200 mg a day -usu in 300-600 mg/daily in divided doses 3 |
PTU Warnings
ADX |
Monitor PT/INR
Preg cat D ADX: agranulocyctosis priuritic maculopapular rash, artharalgia, leukopenia, fever. N&V, epigastric distress, headache, etc with OD |
Methimazole(Tapazole)
A thionamide compound which inhibits biosynthesis of thyroid compounds.
-Same MOA as PTU -10X more potent and acts faster and better tol. than PTU. |
Methiamzole cont:
-Both drugs can induce fetal goiter and hypothyroidisim.
-Long term tx-12-24 months -About same remision rate as PTU |
Methiamzole
Contraindications/ADX |
-Breast feeding/pregnancy
-PTU better for pregnancy -Cross sensitivity between this and PTU ADX: See PTU |
