Mannitol
Osmotic diuretic
use: shock, drug OD, decreases intracranial/intraocular pressure
toxicity: pulm edema, dehydration.
contraind: anuria, CHF
Acetazolamide
Carbonic anhydrase inhibitor (reduces body's HCO3-)
use: glaucoma, urinary alkalinazation, met. alk, altitude sickness
tox: hyperchloremic met acidosis, neuropathy, NH3 toxicity, sulfa allergy
Furosemide
sulfonamide loop diuretic (inhibits Na/K/2Cl cotransporter in thick ascending limb). Increases Ca excretion
use: edematous states, HTN, hypercalcemia
tox: ototoxic, hypokalemia, dehydration, allergy to sulfa, nephritis, gout
Ethacrynic acid
Phenoxyacetic acid derivative.
Same MOA as furosemide, but NOT a sulfa
Use: diuresis
Tox: sim. to lasix, but can be used in hyperuricemia, acute gout.
HCTZ
inhibits NaCl reabsorption in early distal tubule. decreases Ca excretion.
Use: HTN, CHF, idiopathic hypercalciuria, nephrogenic DI
Tox: hypokalemic met alk, hyponatremia, hyper-lipid/uricemia/calcium. sulfa allergy
Spironolactone
Competitive aldosterone receptor antagonist in collecting tubule.
use: hyperaldosteronism, K depletion, CHF
Tox: hyperK, endocrine (gynecomastia, antiandrogen)
Triamterene
Amiloride
Eplerenone
block Na+ channels in collecting tubule
use: hyperaldosterone, K depletion, CHF
Tox: hyperK
Captopril
enalapril
lisinopril
ACE inhibitor, reduces angiotensin II, prevents bradykinin inactivation
use: HTN, CHF, diabetic renal dz.
tox: Cough, Angioedema, Proteinuria, Taste changes, Hypotension, Pregnancy problems, Rash, increased Renin, lower angiotensin II. CAPTOPRIL. hyperkalemia.
