Binds to cholesterol-->toxicity

CAN'T be used systemic, wayy too toxic. Very similar to AmphoB.

BAD TASTE
Flucytosine (Ancabon)

IV or oral
A PRODRUG. Disrupts fungal RNA and protein synthesis. Also for cancer tx (5-FU) --> BONE MARROW toxicity (->anemia/leukopenia/thrombocytopenia)

Good for severe systemics
Griseofulvin (Fulvicin)
Oral. Mitotic spindle poison for fungal infections under nails and scalp.

Best absorbed with fatty meal, enzyme inducer.
Azole drugs
Higher TI than ampho or flucytosine. Inhibit cyto-450 on fungii, blocks precursor to ergosterol formation.

All of them have 3A4 INHIBITION effects!!
Ketoconazole
Azole drug with highest 3A4 interactions.

Will cause increases in: cyclosporine, terfenadine, astemizole, and warfarin

Occasional severe liver toxicity.
Fluconazole
Most common systemic azole prescribed!!! For candidiasis.

Unique: good penetration into the CNS!!

Also fewest p450 problems of the azoles.
Clotrimazole
Miconazole
Topical creams/rectal to treat vaginal candidiasis

Clotrimazole: also for oral candidiasis, better tasting that nystatin.
Enfuvirtide
Fusion inhibitor for HIV tx, binds to HIV prevents fusion to host.
Maraviroc
Binds to T-cell, prevents HIV entry.
Amantadine
Rimantadine
blocks viral uncoating
Acyclovir
Valacyclovir
Val --> Acyclo (it's a prodrug)

For HERPES INFECTIONS
Ganciclovir
needs to be phosphorylated for activation...
Treats CYTOMEGALOVIRUS

100x more potent that acyclo or valacyclo
Supresses bone marrow
Zidovudine - bone marrow
Didanosine - pancreas
Lamivudine
Reverse transcriptase inhibitors

Often cocktailed together.
