What is MC organism for peritonsillar abscess?
Group A strep
How does peritonsillar abscess present? Treat?
unilateral anterior tonsillar bulge w/tonsillar pillar prominent
-UVULA angled towards unaffected side
Tx; drain!! can also give pen or clinda
What are organisms for strep throat? how does it present?
group A strep of strep pyogenes
SS: sore throat, abdo pain, HA, fever
-strawberry tongue, petechiae, exudative tonsils
-vomiting, muscle aches
What are the complications of strep throat?
-scarlet fever
-impetigo
-post-strep glomerulonephritis
-rheumatic fever
Treatment for Strep throat?
Penicillin
MONO: cause? SS? how to test?
Cause: epstein barr virus (EBV) spread through droplets
SS: exudative pharyngitis, ant/post cervical LN enlarge, inc risk spleen/liver enlarge, fatigue
Test: monospot, Ab titier anti EBV IgM/IgG, PCR
Why do you have to avoid contact sports if you have mono?
if the spleen is enlarged, then there is increased risk for a splenic rupture
What is Herpes simplex? SS? Dx?
gums are fire engine red usu 7-10 days (gingivostomatitis) for 1st exposure
Dx: Tzank smear: multinucleated giant cells

white patches on oral mucosa-can wipe off but leaves red spot
-look for diaper rash too
Tx; nystsatin (antifungal)

What will you see if someone has keratoconjunctivitis?
it is from herpes zoster so you will see a dendritic corneal ulcer w/stain
What is hutchinson's sign?
Herpetic lesion on the tip of the nose that means that the eye has been infected
What is the hallmark sign for Retinoblastoma?
Leukocoria (instead of red reflex, you see a white one)
What are the types of neonatal conjunctivitis that you need to worry about?
Gonorrhea: can lead loss of vision
Chlamydia: may lead to pneumonitis
What are SS of allergic conjunctivitis?
-itchy watery-mucous discharge
-cobblestoning mucosa of eye, pale mucosa
-allergic shiners
Tx: antihitamine
How do you diagnose a corneal abrasion?
fluroscein stain
What do you do when a pt has hyphema?
test vision, refer to opthalmology
-keep head elevated to dec scarring & vision loss
What is the best test to diag strabismus?
cover-uncover test
-if eye moves, then there is problem)
What is best test for amblyopia?
it is decreased visual acuity so test for visual acuity
What is the typical presentation of allergic rhinitis?
itchy eyes, pale nasal mucosa, post-nasal drip, pharyngitis
-clear to yellow discharge, conjunctival injection, nose breathers
-allergic salute & shiners
What is the MC organism for sinusitis?
Strep pneumonia, H. influenza, M. catarrhalis (same as Otitis Media)
treatment options for otitis media?
1: amoxicillin
2: augmentin
3: rocephin shots
(azithro is usually not in top 4)
If child is older, is it okay to watch & wait to treat the ear infection?
Yes
When should a tympanostomy tube be used?
4 infections in 6 months
or 6 infection in a year
-if an infection doesn't go away w/multiple courses of Abs
-if there is OME
What are the risks with otitis media w/effusion?
-hearing, speech, learning problems
What does swimmers ear look like?
otitis externa
-swollen, red, tender ear canal
-pain w/manipulation of outer ear & w/exam
- +/- drainage
Mastoiditis: SS? risks? Tx?
SS: post-auricular pain, fever, red//swollen mastoid area
-tenderness to percussion
Risk: spread to organs, meningitis, facial palsy
Tx: hospitalize, give IV Abs
Cholesteatoma: what do you see on exam?
thick greasy mass or pearly white debris w/ or w/o drainage
