Leeway space is:
Difference in the sum of the M/D width of primary 1st and 2nd molars ONLY from the sum of the mesial distal width of the permanent 1st and 2nd premolars. the difference is 2.5mm per side in the mandibular arch and 1.5-2mm (depending on your source) in the max arch.
What is the leeway space of nance?
Difference in the sum of the M/D width of the primary canine, 1st molar and 2nd molar form the sum of the mesial distal width of the permanent canine, 1st premolar and 2nd premolar. That difference is 1.7 per side in the mandibular arch, and .9 mm per side in the maxillary arch.
Why leeway space is needed?
Important to relieve crowding, permanent molars will drift mesially if this space is not preserved.
What is early mesial shift?
The eruptive force of teh permanent molar causes mandibular spaces to close allowing the permanent molar to shift into a class I occlusion
What is late mesial shift?
When no primate space exist, the permanent molar shift occurs when the second primary molar exfoliates in the mandible before the maxilla.
Primate spaces are:
Mesial toe the max canines
Distal to mand canines
Serial Extractions are the first step in
treatment culminating in fixed orthodontic appliance therapy. SHould be an option when crowding is greater than 10 mm per arch. Patient should have a class I dental and skeletal pattern with good lip and incisor position. Guidance of eruption does not correct skeletal problems. The primary canines should be removed first.
Which tooth is removed first in serial extraction?
Primary canine
Is space maintenince active or passive?
Passive treatment. Ortho is active movement
When is space maintenance indicated?
Loss of primary molars, loss of primary canine.
What is a distal shoe?
Used to maintain the space of a primary second molar that has been lost before the eruption of the permanent first molar.
What is ectopic eruption?
Displacement or malposition of erupting tooth, most commonly permanent incisors and molars. Caused by crowding, over-retained primary teeth, missing teeth, infection and caries, development.
What percent of ectopic eruption of permaent molars self correct?
60-66%
What criteria indicate extraction?
Primary molar is mobile, more than 3mm of mesial drift of permanent molar, primary root resorption moderate to severe.
What are anterior and posterior crossbites?
Malocclusions which involve 1 or more teeth in which the max teeth occlude lingually iwth the mand antagonistic teeth.
How do you correct anterior/posterior crossbites?
Reduce dental attrition, improve esthetics, redirect skeletal growth, improve the tooth to alveolus relationship, increase arch perimeter, improve eruptive position (with posterior crossbite correction)
