Neural tube defects refers to defects of the developing:
defects of developing spinal cord, brain, and vertebrae
Why has there been a decrease in prevalence of neural tube defects in recent years?
increased prenatal detection
increased termination of pregnancy increase of folic acid in food*** |
what results if the neural tube does not close completely during gestation?
- encephalocele (malformation of skull)
- anencephaly (lack of development of brain above brainstem) - spina bifida |
what are the different types of spina bifida?
- ocullta (separation of vertebrae)
- occult spinal dysraphism (external mark) - meninocele (external sac- no spinal cord involvement) - memingomnyelocele (external sac- no spinal cord involvement) |
what is the most common type of spina bifida seen in OT?
meningomyelocele
What are the 2 neurological complications of meningomyelocele?
describe purpose of surgery? |
1) chari ii malformation
- spinal cord pulled down - brain herniate through foramen magnum - pressure on cerebellum, uncus - cranial nerve damage 2) hydrocephalus - often seen w/ chari ii - pressure on brainstem stops flow of CSF from lat ventricle to 3rd to 4th etc - results in brain damage surgery; decompress to relieve pressure |
Describe Myelomeningocele
(not the same as meningomyelocele) |
- recall picture of infant w/ a red circle on the spinal cord when they are in prone
- lack of motor/sensation at that point downward - muscles do not develop |
What assistive tool is available for patients with myelomeningocele?
parapodium!
recall that tool where you sway left and right . |
what are the 2 neurological involvements of myelomeninogcele?
1) chari ii malformation
- spinal cord pulls brain down - cerebellar tonsils compress - cranial nerves severed - uncal herniation surgery for decompression 2) hydrocephalus - pressure around 4th ventricle blocks flow of CSF surgery: shunt from lateral ventricles to peritoneum (redirects fluid) |
What are some complications of myelomeningocele?
RISK OF ALLERGY TO LATEX!!
>50% of mmc patients are - higher in children w/ surgery |
OT SAFETY QUESTION! 2 POINTS!
what should you look out for w/ patients with myelomeningocele? |
THEY'RE ALLERGIC TO LATEX
DON'T USE BALLOONS, RUBBER BAND, OR BAND AIDS! and some psychosocial issues (depression) but the latex one sounds way more important for exam |
