Lyme Disease
Clinical Presentation

Stage 1: Erythmea Migrans (Bullseye) Rash


Fever, malaise, headache, arthralgia and stiff neck.




Stage 2: Headaches or stomachaches. Infection of the spleen, eyes, liver, CNS may occur.




Stage 3: Arthritis typically in the knees.

Diagnostic Studies

3 main indications for diagnostic testing is: Erythema Migrans, Facial Nerve Palsy and Arthritis.




Serological testing includes ELISA, IgM and IgG testing; if positive western blot test must be done.

Treatment

Doxycycline or Amoxicillin 1st line of treatment.




  • Less than 8 yr old: Amoxicillin
  • 8 yr or older: Doxycycline
  • If unable to take above: Cefuroxime



DEET to prevent tick bites.



Nonpolio Enteroviruses

Grouped into Human enteroviruses ABCD, Hand-foot-mouth, herpangina, pleurodynia, acute hemorrhagic conjunctivitis, myocarditis, pericarditis, pancreatitis, orchitis,dermatomyositis-like syndrome are manifestations of the infection.

Nonpolio Enteroviruses Skin

Rash may be macular, macular-papular, urticarial (Hives), or petechial.




May look like rash from meningitis, measles or rubella.

Nonpolio Enteroviruses: Herpangina

Often caused by group A coxsackie viruses.


Sudden high fever (up to 106) lasting 1 to 4 days. Loss of appetite, sore throat, dysphagia, vomiting and abdominal in some cases.


Tiny vesicles appear on the mucous membranes in the mouth.


Usually last 3-7 days with complete recovery.



Nonpolio Enteroviruses: Hand-foot-mouth


Disease

Evidenced by fever, vesicular eruptions of the buccal mucosa of the mouth and a maculopapular rash involving the hands and feet.


Rash evolves to vesicles and lasts 1-2 weeks.


Very painful rashes.

Nonpolio Enteroviruses Management

Supportive therapy.

Scarlet Fever


(GABHS)

Pharyngitis


Sand paper rash, often on the groin.


Don't usually have a cough or runny nose.

Scarlet Fever Treatment

The gold standard is PCN, however it tastes horrible.


For children go with Amoxicillin.


If allergic prescribe Zithromax, no taper.
