What are the three diagnostic criteria for acute otitis media?
Signs and symptoms of acute onset, middle ear effusion, and presence of middle ear effusion
How do you identify a middle ear effusion?
bulging TM, limited or absent mobility of the TM, air- fluid level behind the TM, otorrhea
What are the three parts of the ear and what does each do?
1.) External ear: receives sound waves
2.) Middle ear: sound waves are transmitted from air to fluid in the internal ear by small bones
3.) Internal ear: auditory organ in which fluid movements transduce to nerve impulses that travel to the CNS via the acoustic nerve; vestibular organ that helps body maintain equlibrium
What are signs and symptoms of middle ear inflammation?
1.) Distinct erythema of the TM
2.) Distinct otalgia
Should you only address pain only if you decide to give abx for an ear infection?
No, pain should be addressed and managed especially in the first 24 hours of infection if possible
What are some topical and oral agents that are acceptable in treating mild to moderate pain
Tylenol, motrin and benzocaine
When is there an option to treat an ear infection with just observation?
yes, for patients 6 months to 2 years with a nonsevere illness, and for equal to and older than 2 years, as long as there is follow up
What abx should be first prescribed without knowledge of an allergy for otitis media? What is the dose per KG?
Amoxicillin, 80-90mg/kg usually split into two doses; so if a child weighs 100kg, they need a total of 800mg, and if 400/5ml(1tspn) then the child would get 1 tspn twice/day
What is 2nd line for otitis media treatment?
Augmentin
How long should you wait to reassess a patient if otitis media does not improve with treatment?
48-72 hours
What is the clinical presentation of benign paroxysmal vertigo?
abrupt episodes of unsteadiness or ataxia, ?nystagmus, may occur in clusters and may resolve with sleep
