What is orbital vs preseptal cellulitis?
preseptal= periorbital: anterior to orbital septum and outside boney orbit

orbital= postseptal: d/t location being behind orbital septum, orbital infxn of fat and muscle in the boney orbit
What is the most common risk factor for orbital cellulitis?
sinusitis(ethmoid sinuses are covered by lamina propria)
What are the symptoms of preseptal cellulitis vs postseptal?
pain with eye movement, swelling, erythema of tissue around orbit with/without fever
What are causes of preseptal or periorbital cellulitis?
organic infection of soft tissue of face and eyelids secondary to local trauma, insect/animal bites, face bites; strep pneumo, staph, anaerobes are most common
Do you need inpatient treatment for preseptal cellulitis?
- if no improvement after 24 hours, send to hospital; clinical improvement after 48-72 hours means a trial of PO is ok
What are symptoms of preseptal cellulitis?
pain with eye movement more common, chemosis, proptosis, global displacement, limit of eye movement, double vision, vision loss
What is the treatment of orbital cellulitis?
Vancomycin 40-100mg/kg IV per day in 3-4 doses; ceftriaxone 50mg/kg/day or BID
