- symptomatic bradycardia
- complete heart block
- advanced second degree heart block (block of two or more consecutive P waves)

- Patients with syncope who have structural heart disease and inducible sustained VT or VT going to VF on electrophysiology study
- underlying disorders who are deemed to be at high risk for life-threatening VT/VF. This includes: Long QT syndrome, Brugada syndrome, HOCM, Arrythmogenic right ventricular cardiomyopathy
