External Otitis

-definition
-characteristics
Inflammatory disorder of skin on the pinna and/or EAC.

-Edema
-Erythema
-Heat
-desquamation of epithelium
-Exudation
Edema
swelling
Erythema
Redness
desquamation of epithelial cells
dead skin cells
Exudate
puss
External Otitis
Causes
-Bacterial Infection
-Direct exposure to water (constant moisture in small area)
-Allergy
-high ambient humidity
-general skin disorders
-foreign bodies
-trauma
Localized vs. Diffuse
Bacterial External Otitis
LOCALIZED:
-Affects hair cells
-Furunculosis in later stages

DIFFUSE:
-Swimmer's Ear
-caused by constant heat and moisture in small area.
-Chronic Stenosis External Otitis (infection + swelling)
Swimmer's Ear is the common name for what outer ear disorder?
Diffuse Bacterial External Otitis
Complications of External Otitis
1. Furunculosis
2. Keratosis Obliterans
Furunculosis
Painful, red blister on the outer ear.

Associated with External Otitis
Keratosis Obliterans
Cholesteatoma of EAC

-extensive, benign growth of dead skin cells in EAC that excretes destructive enzymes
Outer Ear Viruses
-Viruses damage cartilage and deform pinna

-Ramsay Hunt Syndrome/
Herpes Zoster Oticus

-Bullous myrnigitis

-Chondritis
Herpes Zoster Oticus
Ramsay Hunt Syndrome

definition
symptoms
treatment
-Small, painful blisters on pinna, EAC, and around ear.

-May affect facial and auditory nerves causing central and peripheral issues.
-Tinnitus, HL, vertigo, facial nerve numbness/paralysis
-Absent ARTs, hyperacusis

-Treat with corticosteroids and antiviral treatments
Bullous myringitis
-Infection of TM
-Small, painful blisters associated with URI + otitis media that's moved laterally
Chondritis
Inflammation of cartilage on pinna and EAC.

-Oral antibiotic or topical treatment
Aural Polyps
Inflamed, smooth mass of tissue in the EAC caused by ME infection which develops granulation tissue consisting of vascular connective tissue with pus.
Bacterial Otitis Externa
Treatment
1. Prevention
-No water in ear
-Dry ear after water activity with cotton + vaseline
-swim plugs/ear plugs

2.Kill Bacteria
-Ear drops, neosporin
-Vinegar + acetic acid
-Local treatment 1 week after infection

3. Fever
-Oral Intibiotics
External Otitis- Complications
Treatment
Furunculosis
-Antibiotic steroid drops

Chondritis
-Oral antibiotics
-Topical treatment

Keratosis Obliterans
-Surgical removal
Viral Otitis Externa
Treatment
Incision and drainage of blisters

(most rupture + heal on their own)
