Phacoemulsion is used 99.9 percent of the time these days. There are no sutures, no injections, short procedure. Why would phaco not be the preferred surgery on an extremly dense cataract?
The ultrasound can damage the corneal endotholeium. This leads to more post op edema.
There are two approaches to phaco: clear corneal incision and scleral tunnel. Does CCI require sutures?
No (go in at limbus, faster, no sutures)
This procedure combines a vitrectomy and a lensectomy, and is usually not a planned event. It's used to problem solve, such as if the lens drops back into the vitreous.
PPLX (pars plana lensecomty)
What problem does residual viscoelastic present after surgery?
post of IOP spikess
How long does it take a PSC to come on?
PSCs come on quickly. A dramatic change can result in as little as 12 months.
Why can't you do same day cataract surgery as your evaluation?
the cornea must be "unmolested"
What error are auto Ks on post op cataract sx patients prone to?
too steep
The Zeiss IOL master is accurate within 0.02mm. When should you use an a-scan instead?
If there is a significant interface opacity
Of the IOL power selection formulas: SRK/T, Holladay II, Haigis, Hoffer, which are preferred for very long eyes? which for short eyes?
Long eye -- SRK/T
Short eye-- Hoffer Q
What five data points must be right to obtain the correct IOL power:
1. Desired post-op refraction
2. Average K reading
3. Axial length
4. Anterior Chamber depth
5. Lens constant, the difference between the natural lens plane of refraction and the implant plane of refraction.
What is the lens constant?
The difference between natural lens plane of refraction and implant plane of refraction. Each implant has its own A constant.
You should repeat and do topography if the K readings are?
K<40 or K>48
average K> 1.00D between the eyes
astigmatism>1.50
How much difference must there be between the dioptric power of the two eyes to indicate recalculating the IOL power?
>1.00D
You should repeat the IOL master or do an A scan if the axial length is?
1. <21 or >26
2. axial length difference is greater than 0.3 between the two eyes.
3. axial length doesn' t match the refractive data
When should you recalculate the IOL power?
if the difference IOL power is >1.00D betweem the eyes
What is guttata with corneal edema?
Fuch's
What is a concern in a patient demonstrating PXE (Pseudoexfoliative syndrome)
weak zonules
What is the worst case scenario outcome with cataract surgery?
endopthalmitis
Why does entropion increase the risk of endopthalmitis?
Because the lashes are rubbing on the incision.
