Define oligohydraminos:
volume of amniotic fluid much less than normal
Oligohydramnios is characterized by the following features:
Diminished amniotic fluid volume (AFV)
*Amniotic fluid volume of less than 500 mL at 32-36 weeks' gestation - Amniotic fluid volume depends on the gestational age; therefore, the best definition may be AFI less than the fifth percentile.
*Single deepest pocket (SDP) of less than 2 cm
*Amniotic fluid index (AFI) of less than 5 cm or less than the fifth percentile
Conditions associated with oligohydraminos:
I. Fetal
1. chromosomal abnormalities
2. congenital abnormalities
3. growth restrictions
4. demise
5. posterm pregnancy
6. ruptured membranes
II. Placental
1. Abruption
2. Twin-twin transfusion
III. Maternal
1. Uteroplacental insufficiency
2. hypertension
3. Preeclampsia
4. diabetes
IV. Drugs
1. Prostaglandin synthesis inhibitors
2. ACE inhibitors
V. Idiopathic
Prognosis of oligohydraminos:
Early onset has poor outcome and risk of pulmonary hyperplasia greatly increased
Late pregnancy onset leads to more c-sections for fetal distress
Management of oligohydraminos:
Amnioinfusion is used most often in labor to prevent umbilical cord compression.
