Conventional Ameloblastoma

Histology?
* Mixed cystic and solid areas
* Infiltrative
* Nests and sheets of cells, surrounded by mature fibrous tissue
* Peripheral cells of nests are columnar with nuclei polarized away from basement membrane
* Reminiscent of enamel organ epithelium
* Histo pattern does not affect treatment or prognosis
Unicystic ameloblastoma

Histology?
Luminal: Fibrous wall with cystic epithelial lining
* Cyst lining with focal areas of basal cell nuclei polarized away from basement membrane and with a stellate reticulum
* Not infiltrative into fibrous wall
Intraluminal (plexiform): Nodules of ameloblastoma project into the lumen
* Appears to have interconnecting (plexiform) pattern
* Not infiltrative into surrounding fibrous wall
Mural: Ameloblastoma infiltrates the surrounding fibrous wall
Peripheral ameloblastoma

Histology?
* Same appearance as central ameloblastomas
* May be continuous with surface epithelium
* By definition, should not infiltrate into bone. If it does, it may be a central ameloblastoma that has eroded through the cortex
Ameloblastic carcinoma

Histology?
Cytologic features of malignancy in the primary tumor, recurrence, or metastasis.

Characterized by: increased nuclear/cytoplasmic ration, hyperchromatism, mitoses
Adenomatoid Odontogenic Tumor (AOT)

Histology?
* Well-circumscribed
* Spindle-shaped epithelial cells in sheets or whorled masses
* Rosette areas filled with amyloid
* Tubular or duct-like structures are characteristic
* Mineralized material may look like enamel, dentin, or cementum
Calcifying Epithelial Odontogenic Tumor (CEOT)

aka Pindborg Tumor

Histology?
* Islands, strands, and sheets of polyhedral epithelial cells
* Nuclei may show prominent pleomorphism
* Large areas of hyalinized, eosinic material (amyloid)
* In the amyloid material, get distinctive calcifications with concentric rings (Liesegang)
Ameloblastic Fibroma

Histology?
* Well-circumscribed lesion
* Usually solid tumor
* Cords and strands of odontogenic epithelium
* Surrounded by loose, mesenchyme-like connective tissue with plump fibroblasts
Ameloblastic Fibro-odontoma

Histology?
* Same soft tissue components as ameloblastic fibroma
* In conjunction with a complex odontoma
* Rarely, no enamel is seen, so it is then called an ameloblastic fibro-dentinoma
Central Odontogenic Fibroma

Histology?
* Variable loose and dense fibrous tissue
* May be focal calcification
* Some cases have islands and cords of odontogenic epithelium (these are called the WHO type)
Odontogenic Myxoma

Histology?
* Often erodes through cortical bone and infiltrates in connective tissue
* Gelatinous, loose, myxoid connective tissue with fibroblasts ("ground substance")
* Resembles mesenchyme of dental pulp
* May occasionally see odontogenic epithelial islands
