MCI Defined
an incident with sufficient patient casualties such that additional responding units and resources are required to provide appropriate patient care.
The system is designed to be used with as few as ____ patients and can be expanded to an infinite number as it becomes necessary.

getting the patient to the _______ takes precedence over _______ at the scene when transportation is available.
hospital
treating |
Paramedics will use _______ _________ for treatment to expedite patient care and transportaiton
standing orders
Notify _______ of the incident as soon as possible. The sooner they get notified the sooner that the hospital destinations can be determined
OCC
What are the 5 basic functions that must take place on every MCI
Command the incident
Triage patients treat the patients Load the patients obtain hospital destinations |
as a result of the 5 functions that need to be performed on MCI's. What ICS positions were created?
IC
Treatment unit leader Ground Ambulance Coordinator Med Com |
Depending on the size of the incident one person can hold?
Multiple roles
on smaller incidents the first in captain usually holds two hats. IC and Treatment unit leader. He will then only have to assign MED Com and Ground ambulance coordinator |
The First in company officer must quickly:
assign triage
estimate the number of patients Declare the incident a MCI and establish command Request additional resources consider apparatus and ambulance staging needs |
Triage will be performed using what method
START
simple triage and rapid transport |
Who will report the number of patients as soon as possible?
Triage personnel OR (triage unit leader) will report the number of patients and their triage category to their supervisor asap
When the Incident is declared an MCI notify who? and provide what info?
Notify the agency dispatch center
provide the type of incident , estimated number of patients, and other pertinent info. -take command -Declare an MCI -we have a _______(describe what you see) -with an estimated ____ # of patients |
Upon an MCI being declared whom will the dispatch center immediately notify?
2 people... |
-the dispatch center shall immediately notify OCC and inform them that there is an MCI with estimated number of patients and where it is located
-They will also contact the appropriate (area) B/C |
If Dispatch notifies IC that paramedic resources have been depleted how should patients be transported how?
Consider doing What? |
transport without Paramedics
Consider a central treatment area |
When ordering resources...
immediate patients need ? Delayed patients need? Minor Patients need? |
Immediate- one medic and one EMT
Delayed - one medic and one EMT Minor- only one EMT |
When possible, the following guidelines should be used when ordering Ambulance resources?
_____ Ambulances for every: immediate? Delayed? Minor? |
Immediate- one
Delayed- one Minor- (appropriate transportation) think outside the box, consider contacting OCTA |
Should paramedics that arrive on ambulance transport units be considered treatment teams or transport teams?
treatment teams....NOT transport teams
order additional private ambulances if needed |
When would you use litter teams?
Litter teams will be needed to move patients on incidents that require separate staging and loading areas or treatment areas.
1 litter team = 3 to 4 people |
Who should take the roll of Treatment unit leader?
Is it required to be a paramedic? |
It's strongly recommended that the first in company officer fill this position once the BC takes command REGARDLESS if they are a paramedic or not
What will the treatment leader do?
-Assign treatment teams to patients
-Assign ambulance crews to patients for transport -establish treatment areas if necessary -request litter teams if necessary |
How should paramedics teams should be split for the most efficient use of resources.
they should be split
one paramedic first to each immediate then one to each delayed |
When should central treatment areas be utilized?
only in special circumstances because it requires moving patients twice .
- safety concerns that require pt's to be moved quickly -the scene is dispersed over too large and area to adequately manage -there are not enough PM's to assign one medic to every immediate and delayed patient (if possible use spinal precautions when moving patients) |
When do Litter teams need to be assigned?
- when separate ambulance staging and loading areas are established or treatment areas have been established
What are the duties of the Ground Ambulance Coordinator?
staging and loading of ambulances
- select the ambulance staging and loading areas - Determine an ingress and egress route - request other transportation resources for the minor patients if necessary - request ambulance staging and loading assistants if necessary |
What are the two methods recommended for Ambulance staging and loading?
Combined
Separate |
What is combined Staging?
This is used when ambulances can be staged and loaded in the same place. It's used on smaller incidents with a large area available close to the incident
What is Separate staging and loading?
On larger incidents or in small areas, ambulances need to be staged away from the loading area and brought in when space is available.
How should ambulances be left while in staging or loading patients?
they should be left running and unlocked in case the ambulance needs to be moved
who should be assigned to help the GAC with a separate staging and loading area?
Ambulance staging assistant
- ambulances can be requested each time there is space in the loading are OR - the staging assistant notices the area is obviously clear |
Who will request for public transportation be made through?
Request for public transportation will be made to Control 1 through fire dispatch (is that our metro Net???)
Where should Med Com position himself?
Establish a position forward of Ambulance loading
What will Med Com need to do?
- Contact OCC on 6A & ID yourself as "(incident name)"
- Contact Base OR OCC on assigned talk group - ID yourself as _______ Medcom - provide incident description with estimated # of patients |
IF MED COM is working with a Base hospital what pt. information needs to be relayed?
relay the MCI patient report worksheet information to the base.
(this is acquired from the patient man in the treatment team) |
IF MED COM is working with OCC what pt information needs to be realayed?
If Using OCC, relay the MCI Triage report worksheet info to OCC
What info should MED COM relay to the Amb. driver and what should he be monitoring?
hospital destination
Advise driver to Monitor Med 10 |
What 7 items are included in the MCI patient report
- the transporting ambulance (company and unit ID)
- patient triage category (mild, moderate, acute, MTV or CTV - Patient Age - Patient Sex - Major Injuries - Pulse rate and BP on any MTV or CTV (now just Trauma?) - Destination or specialty center request |
What 3 items are included in the MCI triage report
- identify the ambulance company and ID
- Patient START status classification (minor, delayed or immediate) - Destination or specialty center request |
If the 800 MHZ Radios are down how should contact with OCC be made?
- Med 10
- land line - cell phone - or Radio Amateur Civil Emergency Service (RACES) |
IF the medical communications system is in failsafe and communications with OCC can't be made what should be done?
Med Com will contact the base hospital normally assigned to the medic units assigned in that area.
The base MICN will provide hospital destinations |
If OCC needs to change the hospital destination of the ambulance after they have left the scene how will they do it?
they will contact the ambulance on MED 10
If after leaving the scene the paramedic informs the driver that the destination has been changed, the ambulance driver will notify ____ via _____ __ of the new destination
OCC
Med 10 |
What will OCC do once an MCI has been declared?
- Assign a talk group (6 Bravo-6 Kilo) for use by the med com
- Notify the base hospital normally assigned to the area of the MCI and provide the type of incident, location and estimated # of patients - the base decides if they can handle OR if it's too big they will kick it back to OCC and they will manage coordination of patient destination |
IF the Base hospital is managing the event what two things will OCC do?
- Monitor MCI communications
- Notify the base if the MCI is cancelled |
If the base hospital decides that OCC should manage the event OCC will:
- Alert hospitals to be activated using the Redi Net system
- Activate hospitals will be required to confirm their PCCI(?) and make preparations for receipt of patients - receive info from med com - assign hospital destinations - inform med com of hospital destinations |
Upon Notification of the MCI Dispatch will immediately contact OCC providing what information?
- location of incident
- type of incident - estimated # of patients - any other pertinent info |
