Define psychosis
lack of contact with reality and unaware
may include delusions, hallucinations, disorganization of thought/ behavior/ speech
Define delerium
- altered consciousness due to medical condition, waxes/wanes
Define dementia
- progressive impairment of memory, may have delusions but are an attempt to make sense out of the world so are less bizarre
Mood disorders with psychosis
- psychosis only present with mood disorder
Schizophrenia risk factors and epidemiology
RF~ genetics, prenatal insault, early childhood infection, psycho/social factors
Epidimiology~ 1% population, more males, later onset for females, general onset in late adolescence
Schizophrenia Course/onset
- Some early childhood markers but are not absolute
- few years before onset some non specific mood symptoms
Schizophrenia diagnostic criteria
- 1 month of at least 2: delusions, hallucinations, disorganized behavior, negative symptoms
- 6 months of problems
3 schizophrenia spectrum disorder
1. Schizoaffective- schizophrenia + mood disorder but psychosis present with or without mood disorder
2. schizophreniform~ symptoms less than 6 months
3. Brief psychotic disorder~ less than 1 month and complete recovery
Pathophysiology of schizophrenia
- enlarged ventricles*
- genetic markers
- increased dopamine
Schizophrenia outcome and outcome predictors
- 10-40% chronic
- 25% resolution/benign course
- good outcomes~ female, abrupt onset, late onset
- bad outcomes- mainly negative symptoms or disorganization
What are the 3 other psychotic disorders discussed?
1. Folie-a-deux~ shared psychosis
2. Delusional disorder~ functioning, non-bizarre delusion
3. PTSD~ not truly psychosis, but visual hallucinations/flashbacks
