Fibrocytic Breast Changes:
Proliferative: epithelial hyperplasia (overgrowth of epithelial) more likely to lead to cancer
Non-proliferative: associated with fibroysis and cyst formation. improves with pregnancy and lactation. may disappear with menses.
Fibroadenoma
more common in teens and early 20’s; well delineate, not tender, asymptomatic, rubbery, palpable, mobile. 2-3 cm. slow growth. but dramatic growth during pregnancy
Due to hormonal changes; should not progress to cancer, most times left alone.
biopsy with tissue examination.
tx surgical excision or removal
orcryoablation--insert probe--freeze mass

increase herbal compounds, decrease caffeine.
Intraductal Papillomas:
Tissues in terminal portion of the duct (mammary ducts near nipple area), usual benign (wart like growth) and occurs with in menopause:
nipple discharge.
spontaneous bloody discharge. unilateral mass.
-small ball like lesions that may be detected on mammography, often palpable. typically benign,
excised00papilloma and the duct. one or several ducts can be involved. not linked to breast cancer. age 40-60
Duct Ectasis (comedomastitis)
ducts become inflamed—behind the nipple. thick and sticky discharge--multicolored; pain, burning-itching. may be bloody later. SWELLING
in menopause women, yet not associated with onset of menopause- no risk of CA BENIGN.
cut and remove duct; ABX
may have nipple retraction
TX-SURGICAL EXCISION OF THE DUCTS. NOT NECESSARILY ASSOCIATED WITH CA BUT SHOULD BE REMOVED.
Malignant Tumors
Malignant Tumors--more common in postmenopausal women, usually older htan sevetny. present with pruritus, a lump or flat lesiono r may be relatively asymptomatic.
Vulvar Vestibulitis
Vulvar Vestibulitis-local irritation and inflammation of the vulvar vestibule when there is inflammation there is burning pain and discomfort sometimes itching bc of irritation. could be sexual abuse
Lichen Sclerosus
Lichen Sclerosus-more common in under age 10 and after menopause. benighn plue white papules on teh vulva and the skin around it becomes thin and able to tear and becomes ulcerated and painful. steroids to treat pain
Vitiligo
Vitiligo-depigmented area on vulva and hapens more commonly in complected individuals. No tx. just awareness. not sure about anything!
Bartholin’s Cysts
-an infection of the bartholin glands. could be caused by sti such as gonorrhea or staph. it causes inflammation and pain treat with abx and could cause excise and drain.
vuvlar lesions include
vulvar lvestibulitis, lichen sclerosus, vitiligo, bartholin's cysts.
Endometriosis
Endometri- a tissue found outside of the endometrial cavity.
Cause: Unknown
Symptoms: dull cramping, painful intercourse, tender uterus, may find nodule, abnormal uterine bleeding.
Treatment: NSAIDS: analgesics (danazole), surgery to break up lesions
*non-infectious
anelgesics.
treatment: danazol.-supress endometriosis lupron, nafraelin (supress the menstral cycle) encourage menstruation.
TSS
SS- caused by staph aureus –high absorb and deoderant tampon use diaphragm use left for >24 hr, not washing hands. Characterized by desquimation (peeling) skin on palms of hands and feet, nausea, vomiting, fever >102 deg, diarrhea, myalgia, low BP can lead to disorientation and coma.
tx: Give a vasopressor, may need dialysis. intubation (do bp stuff)
Yeast infections
caused by candida albacons- use of antibiotics, pregnancy, diabetes, oral contraceptives Characterized by itching, thick white discharge, disuria, painful intercourse. Eat yogurt, cotton underwear or no underwear, Monostat or other antifungal agent
bacterial vaginosis
most prevalent vaginal infection in US and world wide. vaginal infection.
--caused by gardnerella vaginalis and mycoplasma hominis
s/s thin watery white or gray vaginal discharge with fould fishy odor

tx. flagyl and geocin

nursing interventions: gentle bathing of vulva prior to applying medicine. education. cotton underware
Trichamoniasis
Trichamoniasis- Infectious STD caused by a protozoan- sexual intercourse, sharing intimate apparel. Symptom frothy yellow/green discharge. Treatment- Flagyl- teratogenic if used in 1st trimester

STD acquired by sexual intercourse and birth caused by a spirocete- Treponema, Pallidum Incubation period is 10-90 days. flu like symptoms. Can be cured if caught early. Found in blood stream. Symptoms- lose of appetite, wart like eruptions on vulva, joint pain, enlarged liver, enlarged spleen liver

tx. tetracycline, erythromycin, penicilins
herpes
TD caused by a virus. Time period between exposure and eruption of vesicles (long blister) can be as long as 20 days. Can rupture and leave ulcer that will heal in 2-4 weeks. type 1 associated with cold sores. Recurrence triggered by suppression of immune systems- stress. Can reside dormant in nervous system. No cure but can use Vit C, zovirax, betadine and acyclovir to treat
chlamidia
- STD can cause PID, infertility, and ectopic pregnancy. Symptoms- thin pyrulent discharge, burning with frequency, lower abdominal pain. Pregnant women treated with erythromycin. Non-pregnant-tetracycline MOST COMMON STD
